Stafford's Evergreen Health Care Center Gets Major Rating

STAFFORD, CT — Evergreen Health Care Center in Stafford has achieved a 5-star rating from the Centers for Medicare & Medicaid Services.

The skilled nursing center increased its CMS Rating this month. CMS combines results from three areas including health inspections, staffing, and quality measures, to get a combined star rating of skilled nursing facilities. The CMS Star Rating is based on a one through five rating scale, with five being the highest. According to CMS, a 3-star rating is considered an average rating for a skilled nursing facility.

Administrator Andrew Goodsell said the Evergreen Health Care Center team holds a "collective vision" for continuous improvement.

"Our commitment to providing compassionate, person-centered care has always been at the forefront of our mission, and this achievement validates our efforts," Goodsell said. "I am proud to lead this team as we will continue to deliver successful outcomes."

Lina Dureza, the director of clinical services for Connecticut, said the center staff has worked tirelessly to achieve this rating.

"The employees are the center’s best and most important asset. They have worked tirelessly to achieve this rating. Their personal actions are reflective of the commitment to always provide excellent care to all the residents," Dureza said.

Goodsell said the team is extremely proud of the 5-Star Rating as staff members are "committed to providing the residents with exceptional care and support to meet each resident’s unique needs."

Evergreen Health Care Center offers short-term rehabilitation and long-term care and is managed by Athena Health Care Systems. It is located at 205 Chestnut Hill Road .
