Senior Health Care Hubs Open At 3 FL Walmarts In New Partnership
Three new Conviva Senior Primary Care centers have opened at Florida Walmart locations.

TAMPA BAY, FL — Conviva Senior Primary Care has partnered with local Walmart stores to open three new centers in the greater Tampa Bay area, according to a news release.
These new centers are among the 23 Conviva and CenterWell Senior Primary Care centers at Walmart locations across four states scheduled to open by the middle of this year.

- 12582 U.S. Hwy 19, Hudson
- 8901 State Road 54, New Port Richey
- 3491 34th Street South, St. Petersburg
The grand opening events at these locations were held April 22-24.
Conviva Senior Primary Care and its sister brand, CenterWell Senior Primary Care, make up Humana’s Primary Care Organization, the nation’s largest and fastest-growing senior primary care provider, according to the news release.

Together they deliver care to about 390,000 seniors in more than 340 centers.
“We’re so excited to bring these three additional Conviva locations to Tampa Bay seniors,” Jacqueline Jarrell, Conviva Market president for Tampa, said. “We work to help eliminate barriers to care by making a visit to the doctor’s office convenient for the community, and it doesn’t get more convenient than next to a Walmart store. Like our other centers, these centers are specifically designed with seniors in mind, including dedicated entrances and easy parking outside with wider hallways, special chairs that quickly convert to examination tables, and wheelchair-enabled weight scales inside. We’re committed to helping our patients lead happier, healthier lives.”
Data shows that patients in “value-based care models,” such as Conviva, see their primary care clinicians 10 percent more frequently, Dr. Angela Ross-Johnson, Conviva’s chief medical office for Tampa, said.
“This means more preventive care and better health maintenance,” she said. “Those Medicare Advantage patients also had 278,000 fewer inpatient admissions in 2023 versus those in traditional fee-for-service care models. This holistic care is what we want for our seniors, and we’re so thrilled to have three more centers open to expand access to our unique care model.”
Conviva Senior Primary Care centers are staffed by board-certified physicians, nurse practitioners, medical assistants and operational support staff, and patients will have access to nurse care coordinators, behavioral health specialists, social workers, a clinical pharmacist, and referral specialists.
The entire team has been specially trained to treat the senior population, the news release said. In addition to its healthcare offerings, many Conviva facilities include wellness centers with games, activities and events.
Conviva locations in the Tampa Bay area accept Original Medicare and a wide range of Medicare Advantage health plans.
