• Observed food items in multiple cold holding units throughout facility failing to maintain food temperatures at 41 degrees F or below (see "temp. observations"). CA: All food times found out of temp were discarded onsite. Legal notice issued and follow-up scheduled.
• Observed prepared food not marked with date of preparation or date for disposal. CA: All ready to eat, potentially hazardous items held for over 24 hours must have a date marking stating the prep date or the discard date.
• Observed hot water not provided/shut off at employee hand wash sink. CA: Hot water was easily restored. Hot water must be accessible at all hand wash sinks.

• Observed main kitchen hand sink blocked by a prep table. CA: All hand sinks must remain accessible.
• Observed raw poultry stored above raw red meat in main kitchen walk-in cooler (improper vertical storage). CA: Items were reorganized onsite. Items must be stored in the correct descending order based on the required final cook temperature of each item.
• Observed items stored in main kitchen walk-in cooler and reach-in cooler failing to maintain temperatures of 41 degrees F or below (see temperature observations). CA: Items observed did not have time to cool and were discarded onsite. All cold held items must maintain temperatures of 41 degrees F or lower. Items that have been cooked have 2 hours to go from 135 degrees F to 70 degrees F and then an additional 4 hours to cool from 70 degrees F to 41 degrees F or lower. Items only have a total of 6 hours to cool from 135 degrees F to 41 degrees F.
• Observed ready-to-eat, potentially hazardous food prepared on site and held more than 24 hours with not properly date marked. CA: Items must be properly date marked and may only be held for 7 days before they must be discarded. Items were discarded onsite.
• Observed hood ventilation system inadequate as evidenced by grease accumulation on walls/ceiling. CA: ventilation hood must be cleaned so as to prevent debris build up.
• Observed two out of three lights above grill area in disrepair. CA: Lights must be repaired.
• A follow-up inspection will be held Oct. 30.
