• Observed black mold-like substance along the interior of ice machine and to-go window ice bin. CA: All food contact surfaces must be cleaned and sanitized. Advised that ice currently held in units be burned and that both units be cleaned as necessary.
• Observed potentially hazardous food cold held at greater than 41 degrees Fahrenheit in main kitchen walk-in cooler #2 (see temp observations). Items without time to cool were discarded onsite. Items with time to cool were placed on ice. Advised that maintenance be performed on unit. Other units in good repair.

• Observed failure to retain most recently obtained shellstock tags/labels for 90 calendar days from the date that is recorded on the tag/label. No tags observed for the months of July or August. CA: Shellstock tags must be retained for 90 calendars.
• Observed several items stored in prep top cooler next to fryer (drawer prep top cooler) and drawer cold holding unit under grill failing to maintain food temperatures at 41 degrees F or lower (see "temp observations"). CA: All items found out of temp were discarded onsite as they had no time to cool. Food must maintain temperatures of 41 degrees F or lower and units must be capable of maintaining adequate food temps, even during peak hours.
• Observed a personal cellphone stored in an area where food could be potentially contaminated. CA: Item was moved to a designated area/corrected onsite. Personal items must be stored in designated areas so as to prevent potential contamination.
• A follow-up inspection will be held Oct. 23.
