Vaccinations, Medical Exam Requirements: Homer School District
Kids in Homer schools have to have certain medical records, based on grade, in the health office before the first day of school.
HOMER, IL — The Homer Community Consolidated School District 33 is reminding parents that students must have all medical files to the school health office prior to the first day of school.
The district follows Illinois' compliance standards, which apply differently to different grade levels, listed below:
Kindergarten:

- Physical exam with immunizations
- Dental exam
- Vision exam
2nd Grade:
- Dental exam
6th Grade:

New students who didn't attend an Illinois school last year:
- Physical Exam with immunizations on an Illinois school form
- Vision Exam
There's one required immunization which must be given after the child's 11th birthday. Meningococcal is required, so if your child has a physical before their 11th birthday, schedule a date for the vaccine and your child will be in compliance, the district said.
