Lincoln-Way Central To Be Vaccination Site Monday

Jewel-Osco Pharmacy will offer COVID-19 vaccinations for ages 12 and up.

NEW LENOX, IL — Lincoln-Way Central High School in New Lenox will be the site for a vaccination clinic offered by Jewel-Osco Pharmacy starting at 3 p.m. Aug. 9. The clinic will be available for anyone ages 12 and over.

"All populations in Illinois 12 years of age and older can receive the vaccine, regardless of insurance coverage or immigration status, including individuals who are 'undocumented,'" a post on the Lincoln-Way Community High School District 210 Facebook page states.

The clinic will offer first and second doses of the Pfizer vaccine. Those under the age of 18 must have parental or guardian signature on their consent form. A parent or guardian must accompany anyone under 16 years old.

All registrants must fill out a consent form before arriving for their appointment. Additionally, all registrants will be required to provide their insurance card or Medicare ID, and driver’s license or state issued photo identification. The consent form can be downloaded by following this link.

Pfizer is a two-part vaccine, with the second dose scheduled for August 30.
