IPS is gearing up to host more immunization clinics at 11 district schools beginning February 9 through March 16.

In addition to providing COVID vaccines to ages 5 and older, health professionals will also provide students with age- and school-required and recommended immunizations, including Hepatitis B, Polio, MMR and others.

Pre-registration and parent/guardian consent is required by visiting https://patients.vaxcare.com/registration.

Use the Enrollment Code IN65942 to begin registration, and then select the name of the school with clinic date. If there is difficulty using the registration link, we encourage trying a different browser (Chrome seems to work best) or submitting a paper Consent Form by scanning via email to clinic@vaccinateindiana.org or faxing to 800-285-1013.

Vaccinations Offered: Note that IIC will vaccinate with all age-recommended vaccines, including the flu vaccine, based on immunization history in the Indiana immunization registry CHIRP (CLICK HERE for 21-22 ACIP recommendations,) and will accept parental refusals of specific vaccines ONLY via completion of the Refusal Form that can be requested by emailing refusal@vaccinateindiana.org or calling 317-628-7116.

IMPORTANT NOTICE REGARDING COVID-19 VACCINES:

  • 1st and 2nd Shots for Ages 5+ and Booster for Ages 12+ will be available.
  • IIC will not initiate the Covid vaccine series without explicit parental request. Requests may be sent via email at clinic@vaccinateindiana.org or by calling 317-628-7116.
  • IIC will complete the Covid vaccine series for any registered student, if it has already been started.
  • If your child has started the Covid vaccine series and you do NOT want them to complete the series, you must email clinic@vaccinateindiana.org within 48 hours prior to the scheduled clinic.
