Join Us at the 2023 CCACC Health Charity Golf Cup

On Friday, June 23, 2023, CCACC Health Center will host its 4th Charity Golf Cup at Richland Golf Club. Proceeds from this event will provide primary care and essential health services for uninsured, low-income Montgomery County residents. In addition, we are also increasing more programs, including:

• S.T.O.P B Project – provides screening, treatment, outreach, and education for Hepatitis B
• Mental Health 360 – provides psychiatric service, psychotherapy, and health education on mental health
• Program to Encourage Active Rewarding Livings (PEARLs) – provides 1:1 case management for depressed, socially isolated seniors in our community
• Integrated Chronic Care Management Program (ICCM) – provides comprehensive care management for patients who have multiple uncontrolled chronic conditions
• Village Program – provides education, counseling, workshop, and peer support group for families with demented loved ones or children with special needs

We would like to invite you to join us this year as a sponsor, register to play as a group or individual, donate cash/products/services, or purchase an ad in our program. Your ad will be displayed in our event program to be distributed to all the golf participants, website, and newsletters following the golf event.
