The Andover Health Division will be hosting a Covid-19 Moderna Booster clinic for residents, age 18 years and over, on Wednesday January 12, 2022, from 10 AM to 1 PM. The clinic will be held in the Select Board Meeting Room, third floor of Town Offices. Appointments are required.  Use the following link to schedule for your appointment.  Please review the following prior to registering for your appointment.

 The clinic is for booster shots only, no first or second doses will be administered.

• If you are unsure of any of the medical questions, please contact your physician.
• Select the best match of your insurance carrier.  It is most important to have the correct member number. 
• Bring your Covid-19 Vaccination Card with you.
