You can get the necessary forms ahead of time on their website: <a href="http://www.dedham-ma.gov" rel="nofollow" target="_blank">www.dedham-ma.gov</a>, the library, Town Hall, or email mlaroche@dedham-ma.gov. A copy of your insurance card must accompany the forms. If you have any questions, please call 781-751-9220.
