ATP Purpose:
Oakland Community Health Network, hereafter known as OCHN, is requesting applications from qualified organizations who provide Vocational Services to eligible adults with mental illness and/or intellectual/developmental disabilities and children with serious emotional disturbance in Oakland County, Michigan.

OCHN invites organizations to submit applications in accordance with the terms and conditions of this ATP. This document provides the requirements and evaluative criteria for Services as described in the Scope of Work section and requests a detailed application from all prospective Organizations in the format specified in the Provider Application (separate document).
