1.0 PURPOSE OF THIS ADDENDUM
Oakland Community Health Network (OCHN) is issuing this Addendum to the Application to
Participate (ATP) issued January 18, 2024.
The following changes shall be made to the ATP:

Language added as shown in red below:
State Approved Training Certificates, as listed on Attachment C, for All Staff Working with
Individuals Served by OCHN (New providers only, if applicable)
Add:
Attachment C (spreadsheet)
Vocational Provide Training Required – Jan 2024
Submit staff State Approved Training Certificates for the trainings listed on Attachment C
and complete the Attachment C spreadsheet. Submit this information with your completed
application.

3.0 TERMS OF THIS ADDENDUM
This addendum constitutes an integral part of the ATP and is to be read in conjunction with
the ATP. Unless specifically changed by this Addendum, all other requirements and terms of
the ATP remains unchanged. It is the responsibility of all Proposers to conform to this addendum.
