OAKLAND COUNTY, MI (February 11, 2025) – Oakland Community Health Network’s (OCHN) vision is to be a champion for the empowerment of persons seeking quality integration of physical and mental health supports and services. This includes youth and adults with developmental/intellectual disabilities, mental health concerns, and/or substance use disorders. We listen and respond to our community’s needs, providing support and opportunities for people to lead independent and self-directed lives. To be responsive to our community’s needs, input from persons served, family members, community partners, legislative leaders, network employees, and contracted service providers is essential.
The organization’s service delivery feedback survey is available for Oakland County residents to offer input through Friday, March 14, 2025. Information gathered from the survey is being used to identify critical service delivery priorities that are person focused. Your support in this important service planning process is greatly appreciated. The responses received will be used to develop OCHN’s annual plan, as well as uphold the organizations mission to “inspire hope, empower people, and strengthen communities”.

About OCHN
OCHN is contracted by the Michigan Department of Health and Human Services to lead a provider service network and manage care for approximately 30,000 Oakland County citizens at more than 400 service sites across the county. People who receive public behavioral health services through OCHN's provider network include those who have an intellectual or developmental disability, mental health concerns, or substance use disorder. Most of these individuals have Medicaid insurance coverage.
OCHN's goal is to ensure these individuals are aware of and have access to services and supports that will improve their health and quality of life, as well as ensure their engagement in full community participation. Its mission to “inspire hope, empower people, and strengthen communities” reflects an unyielding belief in a "Valuable System for Valued People." Programs and supports provided by OCHN's service network are available at www.oaklandchn.org.
