Fill Out Essex County Health Survey
Residents of all 22 Essex County towns are encouraged to fill out this health survey.

ESSEX COUNTY, NJ — The Essex County Executive's Office is conducting a Community Health Assessment to better understand the needs of residents of the 22 towns in the county.
The survey will help the county identify areas of concern, learn about health issues important to residents, and develop a proactive plan for public health initiatives.
Results will be shared with the health officers of all 22 Essex County municipalities.

The survey includes general questions about overall health, diet, and exercise habits, access to affordable medical care and prescriptions, and barriers to obtaining health care.
Assessment participation is voluntary and all responses are confidential.

The township of Maplewood, which shared the survey with residents, said, "We hope that you take a few minutes to complete the on-line questionnaire."
