HRHCare Selected as National Safety Net Advancement Center Grantee
HRHCare will receive more than $83,000 to implement healthcare payment and care delivery reform in under-served communities.

HRHCare, one of the most comprehensive community health care systems in the country, announced Thusday, Aug. 4 that it has been selected as a grantee of the National Safety Net Advancement Center (SNAC) at Arizona State University. HRHCare will receive $83,334 to implement innovative health care payment and care delivery reform projects. HRHCare has been tasked with “Advancing Value-Based Care in New York’s Hudson Valley” – a testament to the unmatched suite of services it offers under-served populations in the region.
This grant will allow HRHCare to enhance strategies for identifying high-cost/high-risk patients, refine its care coordination approach and finalize supportive financial management strategies. Other grantees are based in Alabama, Oklahoma, New Mexico, Illinois and Iowa.
