Nursing Home Inspection Report: Broomall Presbyterian Village

BROOMALL– is a a full-service nursing and rehabilitation center located on Marple Road in Broomall.
The Department of Health and Human Services Centers for Medicare and Medicaid Services filed a survey report on Broomall Presbyterian Village on March 4, 2011.
Broomall Presbyterian Village was given a severity range of D to G, based on eight situations, on a scale from A to L, with A being the least serious and L being the most serious.

A severity score of D through F means no actual harm was done, but there is the potential for minimal harm that does not impose immediate jeopardy, according to Medicare.

According to the report filed, based on clinical record review and staff interview, it was determined that the facility failed to assess the resident when the resident enters the nursing home in a timely manner and failed to make sure each resident received an accurate assessment by a qualified professional.
In addition, the report states the facility failed the following:
- Complete a care plan that meets all of a resident's needs with timetables and actions that can be measured;
- Provide necessary care and services to maintain the highest well being of each resident;
- Make sure that the nursing home area is free from accident hazards and risks and provides supervision to prevent avoidable accidents;
- Store, cook, and serve food in a safe and clean way;
- Have a program that investigates, controls and keeps infection from spreading; and
- Keep accurate, complete and organized clinical records on each resident that meet professional standards.
In regards to controlling infection from spreading, the report states the "facility failed to follow safe infection control practices regarding disinfection of glucometers and failed to ensure that oxygen and nebulizer equipment and one resident's splints were maintained in a sanitary manner on one of four nursing units."
Updates
As of August 21, 2012, no new reports have been filed for Broomall Presbyterian Village in the Pro Publica database. However, a new survey was completed in April 2012.
