Dead Mouse, Live Roaches, Moldy Produce: Ashburn, Sterling Restaurant Inspections

Loudoun County health inspectors found a dead mouse, live roaches and pest droppings at one spot along with other eateries with violations.

ASHBURN, VA — Numerous restaurants in the Ashburn and Sterling area recorded violations during recent health department inspections so far in September. A few had pest-related violations involving rodents and roaches.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation.

Sometimes these are simple, and violations can be corrected during the course of the inspection. Some common violations may include foods hot holding or cold holding at improper temperatures, foods without dates marked, and sanitizer with improper amounts.

Here are restaurant inspection results in and around Ashburn and Sterling in late July from the Loudoun County Health Department. Of note, a Taco Bell location had a dead mouse, live roaches and mouse droppings. Other notable violations were rodent droppings at Aria Restaurant and Manhattan Pizza, moldy produce at Lcuky Strike Sterling and milk past the expiration date at Per Peri Original.

  • Observed several sticky traps uncovered with bait on top. Rodent bait shall be contained in a covered, tamper-resistant bait station.
  • Observed a dead mouse under the hot holding unit. Dead rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
  • Observed two live roaches at different development stages and mouse droppings behind and under all equipment. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by routinely inspecting the premises for evidence of pests.

  • Observed covered container of tuna salad during cooling process. When cooling food should be loosely covered or uncovered during cooling process to facilitate heat transfer.
  • Observed no test kit in facility. Obtain test kit to test the concentration of disinfectant.
  • Observed personal items over single service and single use items rack. Personal items should be stored away from food, equipment, linens and single service and single use items.

  • Observed raw shell eggs stored above carrots. Store food in order of cook temperature with ready to eat foods stored above raw foods.
  • Observed the front handwashing sink in the kitchen not securely attached to the wall. Equipment that is fixed because it is not easily movable shall be installed so that it is sealed to adjoining walls if the equipment is exposed to spillage or seepage.
  • Observed dish clothes stored on top of English muffins. Materials that are used in the construction of food contact surfaces shall not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal conditions shall be safe.
  • Observed two holes where the pipes come out of the wall in the back drink area. The physical facilities shall be maintained in good repair.

  • Person in charge could not locate proper documentation of an employee health policy showing kitchen staff are properly trained to report symptoms of illnesses transmittable through food. Manager must document staff training.

  • Observed raw animal foods with higher cook temperatures (raw chicken stored over raw shell eggs) animal foods with lower cook temperatures in the cooler.
  • Milk container store inside the reach in cooler passed the expiration date. The milk was dated for 9/4.

  • Observed non-food contact safe bleach at the facility. Sanitizer must be safe for food contact surfaces and shall be applied in accordance with the EPA-registered label use instructions.
  • Observed 1 container of chili sauce and a container of powder without a label. Ingredients that are removed from their original packages for use in the food establishment shall be identified with the common name of the food.

  • EHS observed 5 1/4 containers of Heavy Cream with an expiration date of 9/4/2025. Fluid Milk and Milk products must be obtained from Grade A sources. Once the printed expiration date on the container has passed, the product is no longer Grade A. Management voluntarily discarded the containers of heavy cream.
  • EHS observed an employees personal purse stored on the cooking line. Food must be protected from contamination risks by separating personal items from food and food contact surfaces and storing in designated areas away from food and food contact surfaces. Purse was removed from the kitchen.

  • No written procedure or policy for the clean-up of vomit & diarrhea was able to be produced for review during the inspection. A written policy or procedure for employees to follow for the proper clean-up of vomit & diarrheal events. EHS will provide Norovirus clean-up worksheet to management.
  • No signage reminding employees to wash their hands was observed at the front hand sink directly behind the POS system. All hand sinks utilized by employees must have signage reminding them to wash their hands. Provide signage for this hand sink.
  • EHS observed both hand sinks for the facility were blocked by boxes, trash cans etc. and were not easily accessible. Handwashing sinks shall be maintained so that they are easily accessible to employees to encourage frequent hand washing. Manager unblocked both hand sinks during the inspection.
  • EHS observed ready to eat (RTE) soft tortilla shells stored directly on top of raw shell eggs. Foods shall be protected from cross contamination risks by separating raw animal proteins from RTE foods. Tortilla shells were removed to the walk-in cooler during the inspection.
  • EHS observed cut tomatoes and cut lettuce in the cooler located directly across from the grill cold holding at 56F. Time/Temperature control for safety (TCS) foods must be maintained at 41F. Management voluntarily discarded the cut tomatoes and cut lettuce.
  • EHS observed yogurt parfaits in a consumer self service deli cooler with no label information. Food that is made of 2 or more ingredients that is packaged within a food facility must bear a label. Label all yogurt parfaits that are available for consumer self service.
  • EHS observed several mops not in use, stored directly on the ground, and not in the mop sink. After use, mops shall be placed in a position that allows them to air dry without soiling floors, walls equipment etc.

Pizza Hut #37450: 43761 Parkhurst Plaza Ashburn, VA 20147

  • Observed more than 5 boxes of meat stored on the floor in the walk-in freezer. Food shall be protected from contamination by storing the food at least 6 inches above the floor.
  • Observed the three-compartment sink floor drain was backflowing. Person in Charge stated that they had contacted a plumber for necessary repairs. A plumbing system shall be maintained in good repair.

  • Observed spider strainer, fryer baskets, and interior of microwave to have residue on them. Food contact surfaces shall be clean to sight and touch.
  • Observed two containers of sour cream with dates of 7/28/25 and 8/27/24 and a container of heavy cream with a date of 8/31/25. The date of today's inspection is 9/3, which exceeds the manufacturers date labels. Food shall be discarded if it exceeds manufacturer's discard dates.
  • Observed can of lacquer stored co-mingled with food equipment and serving containers. Toxic chemicals shall be stored in appropriate areas so it does not have the potential to contaminate food or food equipment/utensils.
  • Observed several frozen proteins stored in grocery bags. Food shall be stored in containers that are nonabsorbent, easily cleanable, and that does not allow the migration of deleterious substances.
  • Observed multiple knives with damaged handles and pitting along blade. Utensils shall be maintained in a good state of repair.

  • This facility does not have written procedures available for employees to follow when responding to vomiting or diarrheal events in the food establishment.
  • Container of raw shrimp stored improperly over ready to eat sauces in the walk in cooler.
  • The food storage containers for all of the different sauces are visibly soiled with black built up residue around the lids.
  • The following ready to eat TCS (time temperature control for safety foods) observed not date marked for disposition in the walk in cooler: - cooked noodles -cooked bbq chicken - cooked ground meat - Cooked chicken wings The facility is closed on Tuesdays and according to the PIC the above noted TCS foods were cooked and prepared on Monday 9/1/2025.
  • Bottles of employee medicines improperly stored above the prep table with utensils and single service items.
  • Chemical spray bottles observed throughout the kitchen without a label.
  • Bags of vegetables stored directly on the ground in the walk in cooler. Bags of dry goods stored on the ground in the kitchen.
  • Rice scoops improperly stored in deli cup of room temperature water in two separate areas of the kitchen.
  • Facility is reusing single service manufacturer bags for the purpose of food storage. For example a cooked noodles (commercial packaged) used to stored fried chicken in the low boy cooler.

Yum Yum Hibachi: 20706 Weller's Corner Sq #110 Ashburn, VA 20147

  • The following equipment observed in disrepair: 3 compartment sink at bar (water turned off and is not used), bar beer cooler has significant leak, bar hand sink has leak beneath sink and is slow to drain, dumpster lid is broken, and large piece is missing.   • Observed the following equipment surfaces with an accumulation of debris: Fan used in dish room, and refrigeration gaskets throughout facility.
  • The following areas of the physical facilities observed in disrepair: Many small holes in walls throughout facility, light shields in grill area observed cracked, plastic ceiling vent significantly cracked near dry storage cage.

Jersey Mike's Subs: 21031 Tripleseven Rd 160 Sterling, VA 20165

Violations:

  • The general manager on site today during the inspection is not a certified food manager. No employees of this store hold the credential of certified food manager.
  • The person in charge (PIC) was not able to produce a verifiable employee health reporting agreement for each of the food employees.
  • General manager (Person in charge/ PIC) observed washing their hands at the three compartment sink.
  • EHS observed a food employee exit the bathroom and walk in the kitchen to start their shift and immediately start to engage in food prep without first washing their hands.
  • Tomato "Stack" (sliced tomatoes stacked above the cooler line) at the toppings cooler cold holding at improper temperatures. According to the person in charge the tomatoes are served/sold within an hour or two however EHS was not made aware of any time as a public health control policy for said cut tomatoes.
  • EHS observed multiple open deli meats in the walk in cooler not bearing a date mark indicating when the product was opened.
  • Chemical spray bottles observed throughout the facility without labels. According to the PIC the bottles are filled with sanitizer.
  • The PIC was not able to locate the sanitizer test strips for testing the sanitizer concentration at the three-compartment sink.
  • Food residues and grime observed built up in the base of the sliding door meat cooler on the make line. According to PIC the cooler is cleaned out 1 time per month.

El Fresco: 46580 S Cottage Rd Sterling, VA 20164

Violations:

  • When tested, the low temperature dish machine registered at 0ppm sanitizer. Wares not sanitized after cleaning and before use.
  • Rice hot holding at improper temperatures in the steam table next to the rice cooker.
  • Observed raw beef thawing on a shelf at room temperature in the back kitchen.
  • Reduced oxygen packaged fish observed not removed from the packaging when pulled from the freezer for thawing under refrigeration.
  • Facility is utilizing milk crates for food storage in the walk-in cooler (produce) milk crates visibly soiled and not smooth and easily cleanable.

Manhattan Pizza: 21800 Town Center Plaza, Unit #255 Sterling, VA 20164

Violations:

  • The person in charge (PIC) was unable to provide the Environmental Health Specialist (EHS) with a verifiable manner that employees are informed of their responsibility to report in accordance with law, to the PIC, information about their health and activities as they relate to diseases that are transmissible through food.
  • Employees were observed not washing their hands before engaging in food preparation after doing other tasks. Employees must wash their hands immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles.
  • A chemical sanitizer bucket with quaternary ammonium compound (QAC) was found with a concentration of 0 ppm. All QAC chemical sanitizer solutions must have a concentration of 200-400 ppm to ensure proper sanitization of food-contact surfaces.
  • White, Mazbout, Habibi, and Garlic sauces were found in prep unit at temperatures of 70*F. All time/temperature control for safety (TCS) food must be kept at a temperature of 41*F or less to prevent bacterial growth.
  • Facility is removing their sauces from refrigeration and leaving on the counter for customers to easily access. PIC states that they have a policy to only leave sauces out for 4 hours. PIC was unable to provide the EHS with any written policy. A written policy must be in place for sauces to be left out of refrigeration for 4 hours and the sauces must be labeled with the time they were removed from refrigeration to ensure they are being discarded appropriately.
  • In-use utensils were found being stored in a hot water bath that did not exceed a temperature of 104*F. In-use utensils can be stored in a container of water as long as the water is maintained at a temperature of at least 135*F at all times.

  • The water pump was not on and operational at the time of inspection and while employees were engaging in food preparation. Employees must wash their hands immediately before engaging in food preparation and donning gloves.
  • Mobile unit did not have their water pump on and operational at the time of inspection. The water pump must be on and operational at all times during the operation of the mobile unit to ensure the proper number of handwashing sinks are available for employees to use.
  • Boxes of lemons were found being stored on the floor of the mobile unit at the time of inspection. All food must be stored at least 6 inches off the floor to prevent contamination from occurring.
  • Employee sunglasses were found being stored on food-contact surface with clean utensils. Mobile unit must have a designated area where employees can store their belongings without contaminating food, food-contact surfaces, or single-use and single-service items.
