Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. This includes the following: 1) The blades of vegetable peelers have visible food debris. 2) The large strainer hanging on the wall near the 3-bay sink has accumulation of food debris. 3) The preparation tabletops have visible soil accumulation. 4) The interior and exterior portions of the grinding equipment have accumulation of food debris.
  • A large, covered container of chicken curry in the walk-in cooler is 46F. The person-in-charge states that the chicken curry was made yesterday. A large, covered container of onion sauce in the walk-in cooler is 65F. The person-in-charge states that the sauce was made yesterday.
  • Container of cooked mushroom curry is being hot held on the service line at 107F. A container of mango shrimp is being hot held on the service line at 103F. A container of dahl is being stored on the counter at 117F. Time/temperature control for safety food shall be maintained at 135F or above.
  • A container of tomato/mint chutney is being stored at 68F at the service container. Time/temperature control for safety food and shall be maintained at 41F or less.
  • There are several ready-to-eat, time/temperature control for safety foods in the walk-in cooler and preparation cooler that lack the proper date marking or have the wrong date marking. This includes masala, goat curry, yogurt rice, and read sauce.
  • Observed many food containers stored on the floor in the walk-in cooler. This included raw chicken, paneer, sauce containers. Observed two, uncovered containers of lintels stored below the handwashing sink drainpipe on the floor.
  • 1) Observed clean equipment (i.e., cutting boards, pans) stored on the floor below shelves and between equipment. 2) Observed clean equipment stored in the service sink located next to the 3-bay sink. 3) Observed clean equipment stored below the handwashing sink and 3-compartment sink drainboard. 4) Observed boxes and sleeves of single-use cups and containers stored directly on the floor in storage area.
  • 1) The wire shelves above the 3-bay sink that are used to store clean equipment are heavily soiled. 2) The shelving below preparation tables and flame burners are heavily soiled with food debris. 3) The milk crates used to store food off floor are heavily soiled with food debris. 4) The interior portion of the reach-in cooler on the service line is heavily soiled with food debris.
  • Observed heavy soil accumulation at floor-to-wall junctions throughout the kitchen. The walls throughout the kitchen have soil accumulation.
  • 1) A large hole was observed in the ceiling of the women's restroom. 2) A large hole was observed in the hallway wall near the dry storage area.

My Home Thai Bistro II: 19890 Belmont Chase Dr #185 Ashburn, VA 20147

  • EHS observed raw shrimp thawing in a bowl full of stagnant water.
  • EHS observed in-use wiping clothes held in a chemical sanitizing solution (chlorine) at a concentration over the maximum acceptable range > 200ppm.
  • EHS observed heavily damaged test kit used for measuring concentration of sanitizing solution in the three compartment sink. PIC was unable to obtain accurate concentration results using test kit onsite. EHS recorded concentration of sanitizing solution with self provided test kit.
  • EHS observed a sealant around the hand washing sink by the prep table in need of repair. A large portion of the sealant observed missing / ripped off the wall creating a gap with moisture buildup and potential risk for mold.

  • Observed several containers of food that had growth of white substance. This included carrots, beans, bananas, and beets. Several bags of spinach are liquified in the bag and being kept past the manufacture date marking of 11/28/2025. Food shall be safe, unadulterated, and honestly presented.
  • The sanitizer dispenser is leaking and not pulling chemical. The sanitizer at the 3-compartment sink is registering at 0ppm. The facility is actively warewashing. Person-in-charge states that he has purchased a new dispenser and just needs to take the time to install it. Utensils and food-contact surfaces of equipment shall be sanitized before use after cleaning.
  • Food products stored in the walk-in cooler were measured at a temperature above 41F. This included tuna salad (44F) and sliced chicken (44F). Time/temperature control for safety foods shall be maintained at a temperature of 41F or less.
  • Observed clean subway plates stored directly below the paper towel dispenser at the front counter. Water from handwashing drips onto the clean plates. Store clean equipment and utensils in a clean, dry location where they are not exposed to splash, dust, or other contamination.
  • Equipment and utensils are being washed and sanitized but are not being rinsed in-between. The 3-compartment sink is set up with the wash solution in the middle bay. Washed equipment and utensils shall be rinsed so that abrasives are removed, and cleaning chemicals are removed.
  • The drainpipe of the hand sink located near the 3-compartment sink is leaking. 2) The chemical dispenser at the 3-compartment sink is leaking heavily. A plumbing system shall be maintained in good repair.
  • Observed boxes of personal items, unused equipment, and food items not necessary to the operation or maintenance of the establishment stored throughout. The entire mop sink has been blocked by stacked boxes and is filled with items. Employees are unable to access the mop sink. There is debris under dry storage shelves. Boxes and litter are being stored directly outside the backdoor.

  • Bar dish machine was found to have a chlorine chemical sanitizer concentration of 0 ppm when using to clean and sanitize barware. All food-contact surfaces must be sanitized before use and after cleaning.
  • Cheddar cheese that was prepped yesterday, 12/2 was found at temperatures of 45-50*F, lettuce and shredded cheese prepped today (1-3 hours ago) was found at temperatures of 47-60*F in walk-in cooler. All TCS food that is prepared from ambient temperatures must be cooled back down within 4 hours to 41*F or below.
  • Buttermilk dredge, hardboiled eggs, cinnamon butter, whipped butter, and cheddar cheese were found being held at temperatures above 41*F. All time/temperature control for safety (TCS) food must be kept at a temperature of 41*F or below to inhibit bacterial growth.
  • TCS food was found cooling in the WIC while being tightly closed with a tight-fitting plastic lid on the containers. All TCS food that is being cooled must be loosely covered, or uncovered if protected from overhead contamination, during the cooling period to facilitate heat transfer from the surface of the food.
  • Cutting boards and cutting surfaces throughout facility were found to be visibly scored and no longer can be effectively cleaned and sanitized. Resurface/replace visibly scored cutting boards so that they can be effectively cleaned and sanitized.
  • Handwashing sink by dish area and by the back prep line were found to be leaking and in poor repair. A plumbing system must be maintained in good repair.
  • Handwashing sink at the end of the prep line beside the preparation area was found to be not plumbed and going straight into a catch bucket. All handwashing sink and plumbing fixtures must be designed, constructed, and installed according to law.
  • Employee personal belongings were found being stored with food or equipment throughout the facility. Employee personal belongings must have a designated area where they can be stored so that they do not contaminate food, equipment, utensils, linens, and singe-service and single-use articles.

  • Observed the 2-compartment prep sink to be leaking from the faucet and handles. A plumbing system shall be maintained in good repair.
  • Observed a direct connection for the 3-compartment sink and the 2-compartment prep sink. A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed.

Rahama African Restaurant: 45665 W Church Rd #103 Sterling, VA 20164

Violations:

  • Person in charge was unable to produce a written procedure for responding to vomiting or diarrheal events. EHS noted presence of chlorine on site but no written procedure to follow.
  • EHS observed a food employee failing to wash his hands before engaging in preparing carry out order. Prior to starting a task, employee was observed answering phone calls.
  • EHS observed a hand sink by the ware washing area blocked by stationary drying rack and with a stack of wooden rolling pins rested by the faucet.
  • EHS observed in house made and packaged in clear bags sandwiches (large container) stored by the register with a fuzzy moldlike substance visible on the surface. PIC stated the sandwiches are offered for sale to customers as a special item and they were not advertised on the menu.
  • EHS observed a food container storing cooked rice, raw egg and a cup of mayo-based sauce. Raw egg was placed directly on top of ready-to-eat rice. PIC stated the tray contains all ingredients needed for preparing fried rice dish.
  • EHS observed a knife stored on a magnetic strip with a visible residue of dried food particles and other type of soil. EHS also noted a thick grease buildup on top of griddle.
  • EHS observed a tray of goat stew in the walk-in fridge at the improper internal temperature of 56 F. PIC stated the item was prepared yesterday and was cooling overnight.
  • EHS observed deep fried chicken (127F) and fish (108F) stored on hot holding table at improper hot holding temperature. PIC stated both items are fried in batches prior to customer's order due to a high demand and time saving techniques.

  • EHS observed three bags of yuca thawing in a pot filled with water resting in the basin of utility sink.
  • EHS observed prepared and packaged in house containers of yogurt pudding available for customers at grab and go glass door cooler without a proper label specifying contents, allergens and date.
  • EHS observed a container storing baking soda without a clear identification of the material.
  • EHS observed a large pot with raw meat chunks during preparation (marination) process stored directly on the floor near the utility / mop sink.
  • EHS observed a food employee actively preparing food without a proper hair restraint.
  • EHS observed inner and outer walls of microwaves, with an accumulation of food residue and other debris.
  • EHS observed following areas within the kitchen in need of deep cleaning: storage room, mop sink, under counter shelving units, walls behind and between fryers / oven.

  • Person in Charge (PIC) was unable to answer questions of the EHS as it relates to alerting the health department regarding confirmed diagnoses of staff members having a confirmed diagnoses of a food borne illness.
  • Handwashing sink next to food preparation area was missing hand soap and paper towels. Mens bathroom was missing handwashing signage. All handwashing sinks shall be fully equipped with hot water, soap, a means to dry hands, and handwashing signage.
  • Observed utensils stored in cleaned utensils area visibly dirty. Food stored clean shall be clean to sight and touch.
  • PIC informed EHS that refried beans were finished being reheated and two pans were not able to reach the required internal temperature of 165F. Food being reheated shall reach an internal temperature of 165F.
  • Cooked chicken being hot held registered an internal temperature of 126F. Food being hot held shall maintain an internal temperature of 135F or greater.
  • Food being stored on cold holding line was maintaining an internal temperature in a range of 44-45F. Food being cold held shall maintain an internal temperature of 41F or below.
  • Facility is offering undercooked eggs on secret menu while not disclosing the increased risk of consuming raw or undercooked eggs.
  • Kid portion of mac and cheese tightly covered and sealed prior to fully cooling. Food shall be loosely covered, or uncovered if protected from contamination, during cooling process.
  • Observed two cutting boards with deep grooves and scarring in need of either replacement or resurfacing. Cutting surfaces shall be resurfaced at a frequency so they can be effectively cleaned and sanitized.
  • 2 handwashing sinks in kitchen had minimal pressure not sufficient to wash away debris.
  • Observed broken coving tiles in kitchen.

  • Observed heavy dust accumulation on the ceiling vents in the kitchen area. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other material.
