The Loudoun County Health Department found live and dead roaches on glue traps, ants in a buffet station and more violations.
ASHBURN, VA — Numerous restaurants in the Ashburn and Sterling area recorded violations during health department inspections between Dec. 2 and 9.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation.
Sometimes these are simple, and violations can be corrected during the course of the inspection. Some common violations may include foods hot holding or cold holding at improper temperatures, foods without dates marked, and sanitizer with improper amounts of chroline.

- Observed out of order hand washing sink in prep area. At least one handwashing sink, or the number of handwashing sinks necessary for their convenient use by employees in areas such as food preparation, food dispensing, and warewashing areas. PIC walked in with parts to fix the sink.
- Observed can opener and slicer with accumulation of food debris. Food contact surfaces should be clean to sight and touch. PIC had slicer and can opener washed rinsed, and sanitized.
- Observed pink residue on the guard of ice machine. Surfaces of equipment contacting food that is not time/temperature control for safety food shall be cleaned frequency to preclude accumulation of soil or mold. PIC cleaned the guard.
- Observed service date sticker of hood to be service on 8/2024. It has not been serviced since then. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.

- Observed no hand washing sink with signage to remind employees to wash hands.
- Observed raw chicken stored over raw shell eggs in walk in cooler. Food should be stored in a way to minimize cross contamination.
- Observed no date marking on cooked chicken and cooked egg rolls foods that are time/temperature control foods for safety that are kept more than 24 hours should have a date mark.
- Observed medication improperly stored on top of the prep cooler in the kitchen. Medication should be located to prevent the contamination of food, equipment, utensils, linens, and single-service and single-use articles.
- Observed chicken thawing in a container on the counter and raw shrimp thawing on the draining board of the 3 compartment sink. Thawing should be under refrigeration or running water.
- Observed the back with a gap at the bottom and the top of the back door that could allow pests in.
- Observed grocery bag being used and in contact with food.
- Observed cutting boards at prep coolers with heavy scratching and scoring.
- Observed the hand sink with no water pressure.
- Observed the hood system with a large build of grease that looked like greasiciles on the filters.

Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
- Observed low-temp dish machine in kitchen repeatedly measuring 0ppm chlorine sanitizer.
- Observed multiple strip steaks and salmon filets in the cold-prep unit marked 11-26. TCS foods shall be discarded if held past their 7-day use by date with day 1 being the day the food was prepared.
- Observed salmon filets thawing in ROP without any ventilation. Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment: 1. Prior to its thawing under refrigeration as specified in subdivision A 1 of this section. 2. Prior to, or immediately upon completion of, its thawing using procedures specified in subdivision A 2 of this section.
- Observed multiple glue traps throughout kitchen and bar with dozens of live and dead roaches. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
- Observed damages/ missing floor tiles in front of ice machine and in doorway between kitchen and bar. Except as specified under 12VAC5-421-2840 and except for anti-slip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
Lucia's Italian Ristorante & Italian Store: 20789 Great Falls Plaza #188 Sterling, VA 20165

Violations:
- EHS observed an open coffee cup in use at the prep station. Employees may drink beverages while in the kitchen so long as the beverage is closed, and does not pose a contamination risk to foods.
- EHS observed a carton of raw shell eggs stored over a container of cooked ready to eat (RTE) meatballs in the walk-in cooler. Foods shall be protected from cross contamination by separating raw animal proteins from RTE foods and arranging them in a way that mitigates contamination risks from drips, spills, splashes etc. Meat balls were voluntarily discarded and eggs moved to the bottom shelf.
- EHS observed a chef knife with food debris stored on a magnetic strip that also had food debris and soil debris (dust). Food contact surfaces shall be clean to sight and touch.
- HS observed 6 separate containers of cooked pasta cold holding at improper temperatures (52F, 56F, 46F, 46F, 46F, 45F). Time/temperature control for safety (TCS) foods must be maintained at 41F or less. All 6 containers of pasta were voluntarily discarded during the inspection.
- EHS observed 6 containers of cooked pasta with no date labeling in the walk-in cooler. When keeping TCS foods longer than 24 hours, the date that the food item was opened//prepared, or a discard date not to exceed 7 days from the date that the food item was opened/prepared must be labeled on the food/container. All 6 containers of cooked pasta were voluntarily discarded during the inspection.
- Spray bottles at the bar were observed with unidentified liquids within them. When taking toxic materials (cleaners) from bulk supplies, the common name of the material must be labeled on the working container.
- EHS observed a container of cooked eggplant covered tightly with plastic wrap to cool, but was over 135F. When cooling foods, they must remain uncovered, or loosely covered to protect from potential overhead contamination risks so that heat can transfer rapidly. Eggplant was loosely covered.
- EHS observed a package of frozen lobster tails thawing at room temperature on the front deli case at the counter. TCS foods must be thawed under refrigeration of 41F or less, or under cold, continuously running water that is 70F or less. Person in charge moved the lobster tails to continue defrosting in the walk-in cooler.
- EHS observed boxes of raw chicken on the floor of the walk-in cooler and a box of single use to go containers on the floor outside of the walk-in cooler. Food and food contact surfaces must be stored at least 6" off the floor. Move these items so that they are at least 6" off the floor.
- EHS observed a large portion of a tile wall in the prep area of the kitchen that was missing tiles, with exposed cardboard/drywall. Indoor surfaces of kitchens must be smooth, durable and easily cleanable. Repair the portion of the wall that is not easily cleanable or smooth (tile).
Barnes & Noble Booksellers Inc: 20427 Exchange Street - Bld B Ashburn, VA 20147

- Observed rear handwashing sink wrapped in plastic to prevent use. A handwashing sink shall be maintained so that it is accessible at all times for employee use.
- Observed multiple uncovered food containers and trays in the walk-in cooler including rice, cooked chicken, raw chicken, and various sauces. Food shall be protected from cross contamination by storing the food in packages, covered containers, or wrappings.
- After multiple cycles of low temp dish machine, chlorine sanitizer level was observed at 50 ppm consistently, however temperature was consistently too low for effective sanitization, observed at 95 F. Utensils and food-contact surfaces of equipment shall be sanitized before use after cleaning.
- Observed multiple items hot-holding at improper temperatures on the buffet line including tandoori chicken (113 F), fish filets (106 F), and grilled chicken (120F). Except during preparation, cooking, or cooling, or when time is used as the public health control as specified under 12VAC5-421-850 and except as specified in subsections B and C of this section, time/temperature control for safety food shall be maintained at 135°F (57°C) or above. PIC voluntarily discarded the food items.
- Observed multiple items without date labels in the walk-in cooler including rice, cooked chicken, cooked vegetables, and various curry dishes. PIC had employee label all necessary TCS items in the walk-in cooler.
- Observed an open tube of Lucas Oil Red n' Tacky Grease stored on spice shelves. PIC stated this grease is used to lubricate their chain conveyor of their rotating bread oven. This is not a food-safe lubricant. PIC removed the lubricant and discussed food-grade replacement options for future needs.
- Observed multiple flies throughout kitchen, including in dish area and in the unplugged "Supera" reach in cooler". Observed ants crawling on buffet station.
- Observed light shining under kitchen door from exterior dur to damage along bottom of door.
- Observed an accumulation of food debris, rust, mildew, and grease throughout kitchen and walk-in cooler, on floors, all walls, shelving. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
- Observed multiple damaged floor tiles throughout kitchen and walk-in cooler accumulating liquid.

Facility must obtain testing strips for the sink & surface sanitizer. EHS provided correct testing strips to be used until the facility can obtain their own.
- The hand washing sink next to the mop sink in the kitchen is badly leaking and causing water to spray. Plumbing must be in good repair.

- When asked, the PIC was not able to locate a vomit clean up kit or written procedure. - Observed no hand drying provision for the back of the house handwashing sink. Each handwashing sink or group of adjacent handwashing sinks shall be provided with Individual, disposable towels, a continuous towel system that supplies the user with a clean towel, or a heated-air hand drying device. PIC provided paper towels.
- Observed wet wiping clothes on prep cooler. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution. PIC removed wiping clothes and placed it in bucket.
- Observed milk jugs being repurposed for their in-house pistachio milk mix. Single-service and single-use articles shall not be reused. PIC threw away milk jugs and will use reusable food grade containers.

- EHS observed several dirty knives stored on a magnetic strip in the kitchen. Cleaned utensils that have been stored must be clean to sight and touch. Knives were removed from the magnetic strip and placed in the dish pit to be rewashed.
- EHS observed wide rice noodles on the line at 52F and 59F, raw shell eggs at 54F and thin rice noodles at 59F. Reconstituted rice noodles are a time/temperature control for safety (TCS) food. TCS foods must be maintained at 41F or less.
- EHS observed a spray bottle with purple cleaner within that was not labeled. Toxic materials (cleaners) that are taken from bulk supplies must be labeled with the common name of the material on the working container. Person in charge labeled the spray bottle.
- EHS observed sugar, salt and msg outside of their original containers with no label/identifying information. Foods that are taken out of their original container a no are not unmistakenly recognizable must have a label with the common name of the food.
- EHS observed numerous employee items commingled with food items and single use utensils/equipment in the dry storage portion of the kitchen. Food and food contact surfaces of utensils/equipment must be protected from sources of contamination.
- EHS observed several sets of tongs improperly stored on the oven handles on the line. In use utensils that are stored in between uses must be stored on a clean, dry surface, in water that is 135F or above, or in continuously running cold water that is 70F or less and has enough velocity to wash away food particles. Do not store tongs on the oven handles. Tongs were removed from the oven to be cleaned.

- Inner panel of ice machine observed with heavy mildew build up.
- Prepared temperature control for safety (TCS) food item held beyond the disposition date in that tuna salad observed being held in the low boy refrigerator for more than 7 days after being prepared. PIC voluntarily discarded said food item.
- No written procedures exist for the use of time only as a public health control for Cheese Danish.
- Food stored on the floor and/or food stored less than 6 inches off the floor.
- Employees observed working in the food service area without proper hair restraints..
- Wiping cloths improperly stored between use.
- In-use utensils improperly stored between use in that ice scoop observed stored with the handle touching the ice.
- Mold observed on floors and walls in the walk in refrigerator. Walls in food storage area observed dirty with stains.
