47 Health Code Violations Reported At 7 Fredericksburg Restaurants
Rappahannock Area Health District inspectors found health code violations at of the 20 restaurants they visited recently.
FREDERICKSBURG, VA — Inspectors with the Rappahannock Area Health District reported 47 violations at seven of the 20 Fredericksburg food establishments they visited between Feb. 10-21.
In one restaurant kitchen, an inpsector noticed that two large raw ball tip roasts and three bags of raw chicken were thawing in the two-compartment sink.
"Through discussion with the person in charge foods were placed inside the plastic containers at the two compartment sink the night prior to the inspection to thaw," the inspector wrote. "Foods were discarded during the inspection. Environmental Health & Safety discussed proper thawing with the person in charge .

- On the mop sink there is a y-connector that has a long hose attached to one side. The water is on. There is no dual-check valve between the hose and the y-connector, only the bell cap at the back of the faucet. Corrective Actions: The hose was disconnected from the y-connector. I also provided the person in charge with the backflow prevention handout.

Corrective Actions: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
- The wall behind the 3 compartment sink was observed soiled. Corrective Actions: The physical facilities shall be cleaned as often as necessary to keep them clean.
- The filters of the hood ventilation system over the cook's line were observed soiled. Corrective Actions: Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials.

Observations & Corrective Actions
- Observed lack of active managerial control . The person in charge is not monitoring the following : 1. Employee health policy 2. Cleaniness of food contact surfaces 3. Wash , rinsing and sanitizing 4. Cold holding temperatures of TCS food 5. PPM of sanitizer in the dishwasher
- The person in charge was unable to provided a employee health policy . EHS provided a sample to the person in charge.
- Observed a handwashing sink located by the office when turned on no water came out the facuet. During the inspection the person ajusted the facuet and fixed the handsink .
- Observed a food employee weting a wiping clothes inside the handwashing sink near the cooking area. EHS discussed with the employee handwashing sink are for handwashing only.
- Observed the dishwasher in use during the inspection . The dishwasher measured 0 PPM when tested .During the inspection the person in charge reconnect a new container of sanitizer. Sanitizer measured at 50 PPM (corrected ) EHS requsted the person in charge rewash all the dishes that were ran through the dishwasher prior to reconnecting the new sanitizer.
- Observed severel plates and bowl stored soiled underneath the cook line . During the inspection all plates and bowls were relocated to the dishwasher to be cleaned .
- Observed the interior of the microwave and both ovens heavily soiled.
- Observed a food employee at the three compartment sink rinsing and santizing pots and pans only and absent of wash step. During the inspection EHS discussed with the employee and the person in charge wash, rinsing and sanitizing . The three compartment sink was set up for wash, rinsing and sanitizing during the inspection .
- Observed the following foods inside the reach-in cooler under the grill cold holding at improper temperatures : 1. two pans of diced chicken at 45-46 degrees 2. two pans of cut beef at 46 degrees 3. Raw shrimp at 48 degrees 4. Raw sliced steak at 47 degrees Through discussion with the person in charge all foods were relocated from the walk-in cooler at 9:00am the current day of the inspection and placed inside the reach-in cooler. All foods were voluntary discarded during the inspection .
- Observed the following foods improperly thawing at the two compartment sink : 1. two large raw ball tip roast at 53 degrees 2. three bags of raw chicken breast at 54 degrees Through discussion with the person in charge foods were placed inside the plastic containers at the two compartment sink the night prior to the inspection to thaw. Foods were discarded during the inspection .EHS discussed proper thawing with the person in charge .
- Observed the backdoor is cracked open and is not self closing . Observed serveal ceiling tiles missing placed throughout the facility .
- Observed Serveal black Momolcajete not sealed and cleanable used serving foods .
- Observed the following equipment in poor repair : 1. Reach -in cooler under grill is at 48 degrees 2. Reseal around the three compartment sink and handsink facuet 3. Rusted Shelving inside of cooling equipment and by cooking area 4. Ice build up around walk-in freezer door 5. Walk-in freezer door does not close completely 6. Gaskets around the door of the prep cooler and ice machine 7. Three spatulas chipped ( Discarded during the inspection ) 8. Yellow plastic cutting broad broken ( DIscarded during the inspection ) EHS will do a follow up inspection to ensure the reach-in cooler is cold holding properly.
- Observed all non food contact surfaces throughout the facility need cleaning including inside of cooling equipment and exterior of cooking equipment .
- Observed the pipes underneath the three compartment sink are leaking. EHS will do a follow up inspection to ensure plumbing is in good repair .
- Observed in the server area numeruos ceiling tiles with paint peeling .
- Observed throughout the facilities the following in poor repair : 1. Coving damaged 2. Floor tiles cracked 3. Hole in the wall above the walk-in cooler door 4. Wall behind the three compartment sink
- Observed wall ,floors and ceiling throughout the facility (including ceiling air vents ) need a deep cleaning.

- Observed a food employee at the three-compartment sink wash, rinse, and then only rinse the two knives in the quaternary sanitizer solution. The manufacturer's instructions state that utensils/equipment needs a contact time of at least one minute (60 seconds) with the sanitizer solution. Corrective Actions: The employee filled the third compartment with enough sanitizer solution to immerse the knives in it to properly sanitize them.
- Observed meatballs in sauce hot holding at 115°F, instead of 135°F or above. The meatballs were in the unit for approximately one hour. Per discussion with the person in charge they do check to make sure that all of their hot holding items are at 135°F or above. Corrective Actions: The person in charge put the meatballs on the flat top grill to reheated them to 165°F and then put it back in the hot holding unit.

- Observed the interior of the microwave in need of cleaning. Corrective Actions: The cavities and door seals of microwave ovens shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure.
- Observed the interior of the ice machine in need of cleaning. Corrective Actions: Surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned at any time when contamination may have occurred.
- Observed a pesticide repellent device plugged into the outlet on the wall in between the oven and prep unit. Device was removed. Corrective Actions: Insect control devices shall be installed so that: 1. The devices are not located over a food preparation area. 2. Dead insects and insect fragments are prevented from being impelled onto or falling on exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
- Observed heavy ice buildup on the ceiling of the walk-in freezer. Corrective Actions: Equipment shall be maintained in a state of repair.
- Observed the cutting boards on the prep units stained and scratched. Corrective Actions: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
- Observed the following in need of deep cleaning throughout the kitchen: 1) shelving. 2) interior and exterior of equipment. Corrective Actions: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

- Observed a wiping cloth bucket in the front handsink. Item was removed. Corrective Actions: handwashing sink shall not be used for purposes other than handwashing.
- Observed several containers of food in the walk in cooler uncovered. Containers were covered during inspection. Corrective Actions: Except as specified in subsection B of this section and 12VAC5-421-810 B 2, storing the food in packages, covered containers, or wrappings.
- Observed the can opener blade in need of cleaning. Can opener was removed. Corrective Actions: Surfaces of utensils and equipment contacting food that is not time/temperature control for safety food shall be cleaned: At any time when contamination may have occurred.
- Observed a stainless steel container of sliced tomatoes at 46 degrees F in the prep unit next to the cookline. The container was sitting on top of another container with a lid. The container was relocated and had one hour to be used up or brought down to 41 degrees F. Corrective Actions: Time/temperature control for safety food shall be maintained at 41°F (5°C) or less.
- Observed the stainless steel containers in the ware wash area wet while stacked. Corrective Actions: After cleaning and sanitizing, equipment and utensils: 1. Shall be air dried or used after adequate draining as specified in 40 CFR 180.940 before contact with food. 2. Shall not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
- Observed all cutting boards heavily scratched. Corrective Actions: Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
- Observed the sanitizer test strips expired. Corrective Actions: A test kit or other device that accurately measures the concentration in mg/L (ppm) of sanitizing solutions shall be provided and readily accessible for use.Pf
- Observed the following in need of cleaning: 1) shelving throughout. 2) interior of equipment. Corrective Actions: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bibb (threaded faucet) if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by law by: 1. Providing an air gap as specified under 12VAC5-421-2200. 2. Installing an approved backflow prevention device as specified under 12VAC5-421-2210.

- Observed sliced roast beef at 125 degrees F in the hot holding drawer on the cookline. Facility had two hours to use up the roast beef. Corrective Actions: Time/temperature control for safety food shall be maintained: At 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified under 12VAC5-421-700 B or reheated as specified in 12VAC5-421-760 E may be held at a temperature of 130°F (54°C) or above.
- Observed a sanitizer bottle on the prep table where food is prepared. Corrective Actions: Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by: 1. Separating the poisonous or toxic materials by spacing or partitioning. 2. Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles. This subsection does not apply to equipment and utensil cleaners and sanitizers that are stored in warewashing areas for availability and convenience if the materials are stored to prevent contamination of food, equipment, utensils, linens, and single-service and single-use articles.

What Violations Do VDH Inspectors Report?
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
Sometimes, the follow-up inspection only considers the most important "priority" violations.For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
