Fredericksburg Health Inspections: 25 Violations At 1 Restaurant
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Jan. 3 and Jan. 9.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Country Inn & Suites, 4755 James Madison Parkway King George, VA 22485
- Date inspected: Jan. 3
- Total violations: 1
- Observed that the interior of the lower storage cabinet in the kitchen are in need of repair or replacement.
- Holiday Inn Express, 15 Salisbury Drive Stafford, VA 22554
- Date inspected: Jan. 3

- Person in charge could not provide written procedures for bodily fluid cleanup- Document provided during inspection.
- Observed the following hot holding foods not held at proper temperature in the hot holding units on the self service line: cheese omelet (118 degrees F), pork sausage links (122 degrees F), turkey sausage patty (118 degrees F), cut potatoes (115 degrees F). All items were put out 1 hour prior and will be discarded after breakfast in 3 hours. Observed utensils used to pick the food were obstructing the lids of the hot holding units from completely closing. Temperature of foods toward the back of the units were in range. Recommended storing utensils a different way.
- Person in charge could not provide a thin probe thermometer.

- Time is being used as public health control for waffle mix. No certified food protection manager available.
- The person in charge could not provide documentation of training in employee health. EHS provide a document that could be used.
- Observed equipment stored in the handwashing basin. Items were relocated.
- Observed several spray bottles that were not properly labeled. Bottles were labeled during the inspection.
- Observed what appeared to be mice droppings in the cabinet by the homestyle refrigerator. Person in charge instructed an employee to clean the cabinet during the inspection and provided a document showing the facility was treated by a pest control company six days prior to the inspection.
- Observed the following: 1) ice build up in the reach in freezer, 2) cabinet under the three compartment sink is in disrepair, 3) the over the counter refrigerator (43 degrees F) at the self service area needs to be evaluated.
- Observed the ceiling tiles in the kitchen to be absorbent, non smooth, and not easily cleanable.

- The front and rear outer door in the facility are not self-closing.
- China Wok Restaurant, 10617 Courthouse Road Fredericksburg, VA 22407
- Date inspected: Jan. 4
- Total violations: 25
- Absence of a designated person in charge upon arrival to conduct the inspection. The owner arrived within 10 minutes. Ensure a designated person in charge is available during all hours of operation.
- Absence of active managerial control observed during the inspection. The person in charge is not monitoring employee handwashing, ensuring handwashing sinks are accessible, storage of employee supplements, date marking and cleaning and sanitizing of food contact equipment.
- Observed a food employee set-up a sanitizer bucket with Chlorine bleach and then proceed to don single use disposable gloves to handle containers of food. Instructed the employee to discard the gloves and wash their hands.
- Observed the handwashing sink adjacent to the 3-compartment sink is not accessible due to multiple plastic lids filling the basin of the sink. Instructed the person in charge to remove equipment and clean the sink for use by employees.
- Absence of hand drying provision at the warewashing handsink.
- Upon review of the menu, the facility purchases escolar (observed in the walk-in freezer) and is labeled on the menu as white tuna. Update menu to honestly present foods served to customers.
- Observed multiple uncovered containers of cut vegetables in the walk-in cooler.
- Observed the following raw animal foods stored improperly: 1. Uncovered container of pooled unpasteurized shell eggs stored over ready to eat sauces in prep cooler #2 (corrected) 2. Container of raw tilapia stored over an uncovered container of cooked pork in prep cooler #2 (corrected) 3. Container of marinated chicken stored over uncovered container of onions (corrected) and unpasteurized shell eggs stored over covered containers of sauces in the walk in cooler . Instructed the operator to relocate the shell eggs and provided recommendations for storage but this was not completed by the end of the inspection.
- Observed the following food contact equipment soiled: 1. 2 plates on shelving by food production area. 2. Meat grinder 3. Stand mixer bowl and whisk Equipment was removed for cleaning.
- Observed the interior of the microwave oven soiled.
- Through discussion with the operator the following food contact equipment are not being adequately cleaned and sanitized: 1. Food thermometers are being sanitized only. Informed the operator food thermometers are required to be washed and sanitized at least once every four hours when in use. 2. Upon arrival to conduct the inspection, observed food employees cutting raw chicken on the back prep table which was not washed and sanitized after use. Upon discussion with the operator, he reported the tables are sanitized only. Instructed him the table is required to be washed and sanitized. A food employee washed and sanitized the table during the inspection.
- Observed the interior plastic lip of the ice machine soiled.
- Observed the following TCS foods cold holding at improper temperatures inside the top of prep cooler #1: 1- garlic in oil (53 degrees F) and cut cabbage (48 degrees F). Observed the garlic in oil and cut cabbage holding in container above the fill line in the unit and we not being held in refrigeration. A food employee reported the food was pulled from the walk-in cooler at 10:30 a.m. foods were removed to the walk-in cooler to cool to 41 degrees F by 2:30 p.m. Discussed correct cold holding of food in the prep coolers.
- Absence of date marking system in the food establishment for ready-to-eat foods to identify when the TCS foods shall be discarded in the walk-in cooler. Implement a consistent date marking system; handout provided to facilitate training of staff. Through discussion with the operator foods are held in refrigeration for 2-3 days.
- Sushi rice for which time rather than temperature is being used as a control was not labeled or otherwise marked with the 4 hour serve or discard by time. A food employee reported the rice was pulled from hot holding at 11:00 a.m. and placed a note to discard the rice by 3:00 p.m.
- Observed an employee's turmeric supplement stored on a shelf over the rice warmers. During the inspection the dietary supplement was relocated.
- Observed improper cooling methods for cooked breaded chicken in the walk-in cooler. Observed a deep and covered container of chicken that was cooked at 12:30 p.m. Observed the chicken at 82 degrees F at 2:00 p.m.. Worked with the operator to segregate the container of chicken into two smaller batches and placed in the walk-in freezer to cool to 70 degrees F by 2:30 p.m. Observed the temperature of the chicken between 54-38 degrees F at 3:00 p.m. Discussed methods to facilitate rapid cooling.
- Observed 2-3 packages of raw tuna filets and one bag of spicy tuna in the sushi 2 door reach in cooler which were completely thawed in the reduced oxygen packaging (ROP) container which bore a label to remove the fish from the packaging prior to thawing in refrigeration. The operator voluntarily discarded the fish. Discussed correct thawing of ROP fish.
- Observed multiple containers of food stored on the floor of the walk in cooler and freezer which were not part of the delivery received.
- Absence of adequate hair and beard restraints among food employees.
- Observed scoop in the ice well at the soda machine with the handle sitting in the ice. The scoop was removed for cleaning.
- Observed 4-5 egg rolls stored on a soiled paper bag which was being used to line a pan in the 1 door reach in cooler. During the inspection the egg rolls in contact with the paper bag were discarded.
- Observed heavy build-up of debris on the interior sides of the doors of the dishwasher.
- Observed accumulation of debris on nonfood contact surfaces of cooking equipment and shelving throughout the food establishment.
- Observed accumulation of used egg cartons stored on top of soy sauce buckets and on top of the equipment used to roast pork.

- Total violations: 6
- Observed a food employee scrub soapy hands for less than 5 seconds. EHS spoke with food employee regarding the observation and they rewashed their hands properly.
- Observed one container of beef stew ranging from 43-45 degrees F. Per discussion with the person in charge and labeling on the stew, it was made the day prior and food temperatures were not being monitored during cooling process. During the inspection the beef stew was discarded.
- Observed the following in need of repair or replacement: 1. plastic lids on food containers that are broken 2. ice build up in deep freezers 3. rusted shelving.
- Person in charge provided chlorine test strips that expired 5/1/22.
- Observed the following non food contact surfaces in need of cleaning: 1. shelving throughout soiled to touch 2. inside drawers storing utensils 3. outside surfaces of the juice machine
- Observed floors in the dry storage, floors underneath equipment and ceiling tiled dusty and/or stained.

- The cutting board along the prep unit was observed heavily scratched and scored.

- No handwashing signage at the bar hand sink. Signage provided during inspection.
- Observed plastic containers on a shelf not clean to the sight and touch. Items were removed to dish machine for cleaning.

- Observed ready to eat and TCS foods not date marked in the walk in cooler.
- Wiping cloth bucket at the bar service area, when tested with test strips read 200 ppm. Person in charge diluted the chlorine sanitizer to 100pmm.
- Observed a chemical spray bottle in the dish machine area without a common brand label.
- Observed the following in need of cleaning: 1) interior and exterior of all equipment throughout kitchen. 2) shelving throughout kitchen.
- Observed floors underneath equipment, walls, ceiling tiles, and ceiling vents in need of cleaning.
- Observed employee belongings (keys, headband) on shelving where food is stored.
- Taj Indian Cuisine, 2032 Plank Rd Fredericksburg, VA 22401
- Date inspected: Jan. 4
- Total violations: 13
- Observed no paper towels were available in the employees bathrooms bathroom. During the inspection the person in charge provided paper towels.
- Observed the only kitchen hand sink not in proper working order. Employees have to manually turn on the water from the valves underneath the sink. Per discussion with the person in charge a part missing to fix the hand sink has been ordered and will be fixed within the next day.
- Observed 3 containers of samosas and fritters in the walk in freezer uncovered. During the inspection the containers of food were covered.
- Observed the blade of the can opener was soiled. During the inspection the can opener was relocated to the warewashing area.
- Observed the interior surfaces of the microwave were soiled.
- Observed white rice hot holding at temperatures ranging from 114-120 due to insufficient reheating of the rice. Rice was put in hot holding one hour prior to the inspection. During the inspection and before the 2 hours were up the white rice was reheated to 178 degrees F.
- Observed cans of Red Bull and soda stored inside the ice bin at the bar area with ice used for beverages. During the inspection the person in charge removed the beverages.
- Observed food employees in the kitchen working without a hair restraint.
- Person in charge provided an expired test kit (2020) for the chlorine sanitizer used in the facility.
- Observed shelving and handles of equipment throughout the facility require cleaning.
- Walls floors and ceilings of the warewashing area are in need of deep cleaning.
- Observed the following facilities in need of repair: 1. walls in the hallways peeling paint 2. heavily stained ceiling tiles 3. reseal 3 compartment sink 4. wall around mop sink basin is in need of repair and recaulking replaced.
- Lighting of the walk in cooler is insufficient, recorded at less than 3 foot candles.

- Observed icing in the back of the walk-in freezer and one of the toilets in the women's room was leaking. The person in charge advised there is already a work order in for both the freezer and the toilet.
- Observed the following soiled: 1. the upsplash of the drive thru and customer self service drink dispensers; 2. the inside bottom of the prep cooler 2-door near the drive thru; and 3. the inside of the prep cooler 1-door currently not in use.

- Observed raw hamburger patties stored in stainless steel container on a shelf next to and over ready to eat foods in the walk in cooler.
- Observed the can opener soiled. Item was removed to 3-comp sink for cleaning.
- Observed the microwaves on the cook line and server station in need of cleaning.
- The in-use high temperature dishmachine not effectively sanitizing dishes. After running couple cycles, temperature read 149 degrees F and 0ppm when tested for chemical sanitizer. Person in charge will call technician and use 3-comp sink to sanitize dishes.
- Observed several portioned bags of shredded cheese placed in a plastic container stored in a pull-out drawer on the cook line not held at proper cold holding temperatures. (45 degrees F-47 degrees F). The cheese was left in the cooler over night and was discarded during the inspection. Food towards the back of the unit were in range. Person in charge will change from plastic to stainless steel container and fill container under the fill line.
- Observed frozen bag of apples thawing in a pot with still water of 58 degrees F. Item was removed.
- Observed the exterior top lid of grease receptacle heavily soiled.

- Observed three employees preparing food without proper hair restraint. Person in charge provided visors for employees to wear during the inspection.
- Observed several plastic food storage containers which were stacked while wet after washing and sanitizing on the shelving unit near the 3 compartment sink. Items were relocated to air dry properly.
- Observed a hose attached to a Y connector at the mop sink without a back flow prevention device. The person in charge cut the hose to be about an inch above the flood lever rim during the inspection.

- Facility does not have written procedures for the clean-up of vomiting or diarhea events. EHS provided a sample to the person in charge.

- Observed hot water at the front and warewashing handsinks at 60-62 degrees F and the cookline handsink at 85 degrees F. Instructed the operator to advise staff to use the cookline handsink until hot water at the sinks are addressed. Contact plumber to evaluate the hot water by close of business 1/8/2024. Adequate hot water was observed at the 3 vat and food preparation sinks.
- Water turned off at the prep area handsink due to repairs needed. Instructed the operator to contact a plumber to complete repairs needed. Another accessible handsink is available at the cookline.
- Absence of a thin-probed food thermometer in the food establishment to monitor internal temperatures of thin massed foods. The owner reported a food thermometer has been ordered and expected delivery is 1/10/2024.
- Quaternary Ammonium test kit expired July 2019. The owner reported a new test kit is on order, expected delivery on 1/10/2024.

- Observed the caulking at the 3 compartment sink needs replacing, observed black mold like growth on caulking. Also the grouting on coving tiles is missing on wall between grease trap and mop sink.

- Observed a food employee leave the kitchen then return and apply gloves and engage in food preparation without first washing their hands.
- Observed white rice (121 degrees F) stored in a crockpot on the prep table in the back of the house. Rice was re-heated to 165 degrees F.
- Observed a large plastic covered container of pupusa topping (contains cut cabbage) 44 degrees F. that has been stored in the reach in refrigerator by the shelving unit for over 48 hours. The person in charge discarded the item.
- A date marking system is not in proper use for the ready to eat time temperature control for safety (TCS) foods throughout the facility. EHS instructed the person in charge to date mark the items during the inspection.
- Observed employees preparing food without proper hair restraints.
- Observed the following: 1) several broken floor tiles under the mop sink in the back of the house, 2) the wall over the 3-compartment sink is deteriorating.
- Observed employees personal items stored with food products and single service articles.

- Date inspected: Jan. 8
- Total violations: 10
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The soda nozzles on the drive-thru soda machine were heavily soiled. When asked how often they clean them, the person in charge said that they clean them at least once per week. Person in charge was instructed to wash, rinse and sanitize the nozzles at least every 24 hours. Nozzles were cleaned during inspection.
- Observed bags of English muffins on the floor. Muffins were re-located during inspection.
- Observed employee wearing a watch while preparing food. Watch was removed during inspection.
- Observed two employees with beards preparing food without beard guards. Also observed another employee with their hair hanging below and in front of their shoulders.
- Observed cases of single service stored on the floor in the dry storage area.
- The following non-food contact surfaces were observed in need of cleaning: 1-The outer surfaces of the dish machine, 2-the surfaces of the hot holding drawer unit, 3-the upsplash of the coffee maker, 4-the upsplash of both the dining room and the drive thru soda dispenser.
- Observed a large amount of standing water near the mop sink.
- The following physical facility items were noted in need of cleaning: 1-the HVAC vent above the dish machine (mold), 2-the floors in the dry storage area.
- The ventilation hood above the deep fryers were noted in need of cleaning.
