Leesburg Health Inspections: Living And Dead Cockroaches

Several restaurants around the Leesburg area recorded violations during the most recent round of health department inspections.

LEESBURG, VA — Several restaurants around Leesburg and western Loudoun County recorded violations during recent inspections from the health department. Inspectors conducted the most recent restaurant visits between Sept. 20 and Sept. 26.

COVID-19 restrictions around the state have relaxed, so health inspectors have returned to conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • Bhai Sahab, 19405 Promenade Dr Bld L Ste 106 Leesburg, VA 20176
    • Date inspected: Sept. 20
    • Total violations: 7
      • Observed plastic containers be used to scoop for sugar and lentils. Food shall only contact surfaces of utensils that are cleaned.
      • Observed several cleaned knives ready to be used with food residue on the blade. Utensils should be clean to sight and touch.
      • Observed improper cold holding temperatures unsalted butter 65F on counter. Foods held cold should be maintained at 41F or below.
      • Observed chicken cooked earlier now cooling in the walk in cooler in a container with a lid tightly covering the chicken. When cooling foods the food should be loosely or uncovered and in a way to minimize cross contamination.
      • Observed live cockroach crawling on the wall in the kitchen. Facility has pest control and last visit was September 2023.
      • Observed cutting board on prep cooler with heavy scratches and scaring making the cutting board no long easily cleanable.
      • Observed ice machine plastic guard with a build up of dirt and debris. Non food contact surfaces should be cleaned at a frequency necessary to preclude accumulation of soil residues.

    • Total violations: 1
      • Employee handwash sink in kitchen (front) does not have signage instructing employees to wash hands.
  • Lovettsville Diner, 2 N Berlin Pike Lovettsville, VA 20180
    • Date inspected: Sept. 20
    • Total violations: 4
      • Observed raw shell eggs stored over ready to eat produce in the 2 door refrigeration unit. Ensure raw animal proteins are stored in a manner that prevents potential cross contamination.
      • Observed no sanitizer at the 3 compartment sink while facility was doing dishes. Sanitizer tested and recorded at 0 PPM Quaternary Ammonium. Ensure wares are being properly sanitized.
      • Observed tuna salad with a prepared by date of 9/12 in need of disposition. Food has exceeded the 7 day shelf life, with day 1 counting as the date the food was prepared.
      • Observed time as a public health control rather than temperature control being used for various items in the kitchen not marked or otherwise identified to indicate the time when the food was removed from temperature control. Food items included 4 trays of shell eggs and scrapple.
  • 1836 Kitchen & Taproom, 34 E. Broadway Street Lovettsville, VA 20180
    • Date inspected: Sept. 20
    • Total violations: 5
      • Observed no verifiable employee health policy at time of inspection. Food employees and conditional employees must be informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
      • Observed bar hand washing sink with the hot water turned off. A handwashing sink shall be equipped to provide warm water through a mixing valve or combination faucet.
      • Observed bar dish machine not properly sanitizing wares. Bar dish machine was ran multiple times and tested. Dish machine recorded at 0 PPM chlorine with a maximum surface plate temperature of 131F.
      • Observed grits (119F) and creole sauce (120F) hot holding at improper temperature in the hot box. Ensure time/temperature control for safety foods are hot held at 135F or above.
      • Facility was not able to locate a test kit for Quaternary Ammonium utilized at the 3 compartment sink as sanitizer. Ensure facility has a test kit to accurately measure sanitizer concentration.
  • Goodstone Inn(Conservatory), 36205 Snake Hill Road Middleburg, VA 20117
    • Date inspected: Sept. 20
    • Total violations: 2
      • Verifiable employee health policy not available for food handling employees. Each food handling employee needs to have signed a verifiable employee health policy which indicates training in the signs and symptoms of foodborne illness.
      • Container of olives, sliced, in walk-in, has exceeded 7 day shelf life.
  • Fireworks Pizza, 201 B Harrison St Leesburg, VA 20175
    • Date inspected: Sept. 21
    • Total violations: 6
      • Observed employee cut and handle a meatball sub (Ready-to-eat) with their bare hands. One hand of the employee contained a single service glove and the other did not. Food employees shall not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment.
      • Observed hand washing sink at bar to have a strainer in the basin of the sink. When asked, employee stated that is the location where leftover drinks are dumped. Ensure hand washing is used for hand washing only. No dumping of beverages is to be done in the hand washing sink.
      • Observed white sauce on ice cold holding at improper temperature (55F). Ensure time/temperature control for safety foods are cold held at 41F or below.
      • The following method used for cooling was not adequate to facilitate proper cooling: placing warm cooling foods in a container and placing a tight-fitting lid on the container. Several food items were found in the walk in refrigeration unit with tight lids, not having completed the first part of the cooling process.
      • Observed cutting board on the small prep unit heavily scratched and scored. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced.
      • Facility could not locate test kit to test for chlorine sanitizer for the dish machine. Ensure facility has a proper test kit to measure effectiveness of chlorine sanitizer used for the dish machine.
  • Hamilton Station Gastropub, 17416 Hamilton Station Rd Hamilton, VA 20158
    • Date inspected: Sept. 22
    • Total violations: 4
      • There is no employee in this facility with a certified food protection manager certificate. At least one employee with supervisory and management responsibility and authority to direct and control food preparation and service shall be a certified food protection manager.
      • Several items in the refrigeration units (dolmades, crab dip portions) do not have date marking indicating when the TCS item should be served, sold or discarded.
      • In walk in refrigerator (packaged beef) and in dry storage (panko bread crumbs and potatoes) are not elevated food 6" above ground. Store food items (dry storage and refrigeration 6" above ground to prevent contamination.
      • Wet wiping cloths observed on food prep surfaces. Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.

    • Total violations: 1
      • Dead cockroaches (3) observed below the dish machine in the ware washing area. Improvement observed in pest management however efforts should be continued to be made daily.

      • At follow-up inspection, the dish machine has not been repaired. The dish machine was tested and recorded at 0 PPM Chlorine.

    • Date inspected: Sept. 25
    • Total violations: 2
      • Miscellaneous items (lubrican, cleaning tools) found in and around employee handwash sink closest to dishmachine area.Use handwash sink only for handwashing. Person in charge removed miscellaneous items from employee handwashing sink. Corrected.
      • Irreversible temperature measuring device is not available to monitor the surface temperature of wares being washed, rinsed and sanitized in high temperature dish machine. Minimum surface temperature : 160F.

    • Total violations: 1
      • Employee in kitchen does not have a verifiable employee health policy which represents that the employee has been trained in the signs and symptoms of foodborne illness. Each food handling employee needs to have a verifiable employee health policy indicating they have been trained in the signs and symptoms of foodborne illness. Environmental Health Specialist provided a copy of Form 1B Employee Health Policy.
