Leesburg Health Inspections: Malfunctioning Dishwasher, Pesticides

Several restaurants around the Leesburg area recorded violations during the most recent round of health department inspections.

LEESBURG, VA — Several restaurants around Leesburg and western Loudoun County recorded violations during recent inspections from the health department. Inspectors conducted the most recent restaurant visits between Jan. 3 and Jan. 9.

COVID-19 restrictions around the state have disappeared, so health inspectors have returned to conduct most restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • Shake and Bops, 19 Wirt St SW Leesburg, VA 20175
    • Date inspected: Jan. 3
    • Total violations: 5
      • At time of inspection, person in charge (PIC) was unable to locate a verifiable employee health policy that informs employees of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
      • At time of inspection, person in charge (PIC) was unable to provide written procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.
      • Observed no hand washing cleanser available at the hand washing sink by the 3 compartment sink. Sink was also observed with a wooden piece to match the counter in the basin of the sink, making the sink not accessible. Ensure hand washing sinks are supplied and accessible at all times.
      • At time of inspection, person in charge was unable to locate a food thermometer (stem thermometer) to use in ensuring attainment and maintenance of food temperatures. Food temperature measuring devices shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
      • At time of inspection, facility was unable to provide test kit to measure quaternary ammonium sanitizer utilized at the 3 compartment sink. A test kit or other device that accurately measures the concentration in mg/L (ppm) of sanitizing solutions shall be provided and readily accessible for use.

    • Total violations: 3
      • Observed kitchen hand washing sink to not have a hand drying provision. Paper towel dispenser observed empty. Ensure hand washing sinks are supplied with hand drying provision at all times.
      • Observed kitchen hand washing sink to not be supplied with hand washing cleanser. Ensure hand washing sinks are supplied at all times with hand washing cleanser.
      • Observed chemical dish machine to have 0 PPM chlorine at time of inspection. EHS observed sanitizer line disconnected. Sanitizer line was running sanitizer on the floor of the kitchen instead of into the machine. Ensure wares are properly washed, rinsed, and sanitized.

    • Date inspected: Jan. 4
    • Total violations: 2
      • Observed kitchen hand washing sink not supplied with a hand drying provision. Paper towel dispenser was observed empty. Ensure hand washing sinks are supplied at all times with a hand drying provision.
      • Observed containers of food stored on the ground in the walk in refrigeration unit. To prevent potential contamination, ensure food is stored at least 6 inches off the ground.
  • T'Kila Mexican Kitchen & Bar, 19345 Promenade Dr Leesburg, VA 20176
    • Date inspected: Jan. 4
    • Total violations: 6
      • Observed mayonnaise on the bottom shelf with raw chicken. Raw shelled eggs on top of a beer case. Ready to eat food should be separated and protected from raw animal food to prevent cross contamination. Person in charge (PIC) relocated the mayo to a ready to eat shelf and moved the eggs to the bottom shelf next to the raw chicken.
      • Observed low temperature dishwasher not sanitizing properly. When testing the dishwasher with chlorine test strips it registered 0 parts per million (PPM). Maintain 50-100 PPM in dish machine for proper sanitization. The three compartment Quaternary Ammonia sanitizer was tested and registered 100 PPM. Maintain 150-400 PPM Quaternary Ammonia for proper sanitization. PIC called their maintenance company to recalibrate both dishwasher and three-compartment sanitizer dispensers.
      • Observed soiled can opener by prep table. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC placed can opener to be washed in three-compartment sink.
      • Observed raw animal food stored on the floor. Food should be stored at least 6 inches (15 cm) above the floor. PIC relocated food off the floor to shelves in walk in cooler.
      • Observed bowls in several bulk containers. Utensil can be stored in food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins. PIC removed bowls and will supply bins with scoop with handles.
      • Observed warped and soiled cutting board next to the grill. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they are not capable of being resurfaced. PIC voluntarily discarded the cutting board and replaced it with another cutting board.
  • New Yorker Pizzas, 828 South King St Leesburg, VA 20175
    • Date inspected: Jan. 4
    • Total violations: 5
      • Observed hand washing sink in the back area of the kitchen blocked by pans. Ensure hand washing sinks are used for hand washing only and that the hand washing sink is accessible at all times.
      • Observed no hand drying provision at hand washing sink in the back area of the kitchen. Ensure hand washing sinks are supplied at all times with a hand drying provision.
      • Observed an uncovered container of raw chicken stored directly on ready to eat produce. Ensure raw animal proteins are stored in a manner that prevents potential cross contamination with ready to eat foods. Ensure stored containers of food are protected by wrappings or coverings.
      • Observed facility to have non-commercial pesticide in the facility behind the stand mixer. Observed one container of Raid Ant & Roach Spray, two containers of Raid Fogger, a gallon size container of Indoor insect barrier, and several white, tablet size substances behind the stand mixer spread along the ground. EHS could not verify what these items were at time of inspection. A restricted use pesticide shall be applied only by an applicator certified as defined in 7 USC § 136(e) (Federal Insecticide, Fungicide and Rodenticide Act), or a person under the direct supervision of a certified applicator. If utilizing bait, rodent bait shall be contained in a covered, tamper-resistant bait station.
      • Observed several utensils in stagnant, room temperature water on the flat top grill. If the facility chooses to store in-use utensils in water, ensure the water is maintained at 135F or has running water of sufficient velocity to flush particulates to a drain.
  • Mr. Sushi & Mrs. Roll, 19302 Promenade Dr Ste #103 Leesburg, VA 20176
    • Date inspected: Jan. 5
    • Total violations: 3
      • Handwash sink water splashes onto plates and a cutting board adjacent to the handwash sinks. Food contact surfaces are required to be clean any time during the operation when contamination occurs. Person in charge corrected by cleaning the cutting board and dishes. These items were relocated to a different area away from the handwash sink.
      • Two bulk containers of soap and oil/greaser for equipment was found stored on the dry storage shelf above tempura batter and single use items. Chemicals are required to be stored below and away from foods and single service items to prevent contamination. Person in charge corrected by moving chemicals to the bottom shelf.
      • Observed employee clothing placed on the dry storage shelf next to single-use items and food. Designated areas such as lockers, or other suitable facilities are required for the storage of employee clothing and possessions. Person in charge corrected by removing employee items from the dry storage shelf.
  • North Spring Behavioral Healthcare, 42009 Victory Lane Leesburg, VA 20176
    • Date inspected: Jan. 8
    • Total violations: 1
      • Observed kitchen hand washing sink near the 3 compartment sink unable to provide cold water. When cold water nozzle is turned on, hot water is dispensed. Ensure plumbing is maintained in good repair and hand washing sinks have hot and cold running water.

      • This food establishment does not have procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • There is no irreversible thermometer or thermo test strips to monitor the hot water from the hot water dish machine to be at a minimum of 160F (surface).

    • Total violations: 1
      • Multiple spray bottles of cleaners were observed around food prep, storage area near front line. Person in charge relocated spray bottles below employee handwash sink storage area. Corrected at inspection.

      • Observed a layer of cardboard on top of storage shelf above prep table in kitchen. Non food contact surfaces need to be cleanable. Person in charge removed cardboard layer. Corrected at inspection.

    • Date inspected: Jan. 9
    • Total violations: 5
      • Not all employees have been trained in the signs and symptoms of foodborne illness. There needs to be verification (employee health policy form 1B) that each foodhandling employee has been trained in the signs and symptoms of foodborne illness.
      • No hand towels are available at the handwash sink next to the 3 compartment sink. Provide hand towels at each hand wash sink.
      • Sausage patties observed hot holding below minimum hot hold temperature of 135F. Person in charge reheated sausages to 183F. Corrected at inspection.
      • Pancake batter was observed at 67F. Maintain TCS (time/temperature for safety foods) at 41F or below. Person in charge returned pancake batter to refrigeration to cool.
      • Accumulated debris at the juncture of floor/wall in kitchen, under 3 compartment sink and behind stainless equipment throughout kitchen. Clean the physical facilities at a frequency necessary to maintain clean to sight and touch.

    • Total violations: 1
      • Observed hotel pan of raw turkey breast improperly stored on a shelf directly over a hotel pan of commercially processed fully cooked beef pot roast.
