Self-Management Health Programs Free

Independence at Home (A SCAN Community Service) and the Self-Management Resource Center are offering two free 6-sessions classes this fall.

Free in-person classes are coming to Long Beach courtesy of Independence at Home (A SCAN Community Service) and the Self-Management Resource Center.

The two in-person series will take place a the Molina One Stop Help Center (6583 Atlantic Ave. #110, Long Beach 90805) and are each six sessions.

Note: Participants do not need to be a member of SCAN Health Plan nor Molina Healthcare to attend.

Chronic Disease Self-Management begins Tuesday, October 7, 2025, and runs consecutive weeks until November 11, from 10 am until 12:30 pm. Participants will learn about the importance of healthy eating and physical activity, how to manage medication, control emotions, and how to effectively communicate with your doctor. The classes empower people living with chronic diseases to create an action plan, to solve problems, and make good decisions.

Diabetes Self-Management Program is also offered at no-cost and is specifically for people (ages 18+) living with Type 2 diabetes. The classes are from 1:30-4:00 pm on the following Thursdays: October 30, November 6, 13, 20, December 8, and 18 (no class November 27 in observance of Thanksgiving and December 11).

SCAN Independence at Home (IAH)

Dedicated to helping older adults and their caregivers, Independence at Home is SCAN's commitment to its nonprofit mission of keeping seniors healthy and independent, while supporting communities, by offering crucial support and services.
