Patient Dies After Fall in Middletown Nursing Home: Report

Middletown's Water's Edge Center of Health and Rehabilitation was fined after a patient within days after a fall.

Water’s Edge Center of Health & Rehabilitation in Middletown was fined $1,160 on Dec. 2, 2015, in connection with a resident who died within days of falling out of bed at the home on Nov. 16, according to Kate Farrish of the Connecticut Health Investigative Team.

“Though a care plan called for two staffers to turn the resident in bed, only one nurse’s aide was turning the resident when the fall occurred, DPH’s citation said,” the report by Farrish stated. “The resident was bleeding heavily and hospitalized with broken facial bones and a broken spinal bone, DPH’s citation states. The person was transferred to a hospice unit and soon died.”

An investigation was conducted into the incident and the aide responsible was fired for not following the care plan.
