St. Thomas Church Holds Advanced Care Planning Seminar
Daun Barrett, RN will be speaking at St. Thomas Church on Apr. 30 to discuss the three stages of Advanced Care Planning.

From St. Thomas Apostle Church: Does your caregiver know what your healthcare decisions are? On April 30 at 1 p.m. Daun Barrett, RN, director of Community Outreach & Parish Nursing at Griffin Hospital, will present a free discussion on the three stages of Advanced Care Planning at St. Thomas the Apostle Church, 733 Oxford Road. Advanced Care Planning is a process of understanding, reflecting, and discussing individual goals and value’s to determine one’s overall wishes should a time come when one cannot make one’s own medical decisions and is unlikely to recover from an injury or illness. Advanced Care Planning is the best way to make sure your healthcare wishes are respected by your loved ones and healthcare providers. This process also gives comfort and reassurance to one’s healthcare representative knowing that he/she clearly understands one’s end-of-life preferences. To register, please call Maryjane at 203-888-2382 ext 121
