- Observed previously served foods from buffet stored in large reach-in cooler (front). CA: All items removed from buffet must be discarded. COS: Discarded. Corrected On-Site. New Violation.
- Observed wiping cloth sanitizer solution at very low levels (0-50 ppm CI). CA: Ensure sanitizer has the proper level. COS: Remade to 50-100 ppm CI. New Violation.
- Missing hand washing signage in restrooms. CA: All handwashing facilities must adequate signage. New Violation.
- Improper reuse of containers for food storage some had holes drilled. CA: Only use properly designed food containers. COS: Buckets discarded and items moved. New Violation.
- Observed lid for ice machine, various shelving and some storage racks in disrepair/not clean. CA:Clean/fix. Repeat Violation.
- Continue making needed repairs to floor/walls throughout kitchen. Repeat Violation.

- No response procedures in case of illness event in restaurant. Handout has been previously provided. Policy must be available on request. Repeat Violation.
- The food slicer (used for slicing various vegetables) shows food debris from not being properly dissembled and cleaned. CA: Food contact surfaces must be properly cleaned before storage or every 4 hours. COS: Cleaned. Corrected On-Site. New Violation.
- Improper reheating procedures observed for the queso, product was stored on top of the oven at a temperature of 70 deg F before being placed into the steam well. CA: Reheating for hot holding shall be done rapidly and the time the food is between the temperatures of 41°F (5°C) and 165°F (74°C) and 41°F (5°C) and 135°F (57°C) for commercially processed food, may not exceed 2 hours. COS: Reheated to 165 deg F. Corrected On-Site. New Violation.
- Ice scoop handle stored in contact with the ice. CA: .In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon etc. COS: Moved. Corrected On-Site. New Violation.
- Small cup used as a scoop in rice. Small cup used as a scoop in sugar. CA: Only use properly designed scoop with handles. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition COS: Discarded. Corrected On-Site. New Violation.
