ICYMI: Harris Five Forks Cafe Fails Inspection

How did your Bartow County favorites perform on their recent food service inspections?

Here are some recent food service inspections conducted by the Bartow County Environmental Health Department:

Harris Five Forks Cafe (Food Service Inspections)*

View inspections: December 20, 2018 Score: 65, Grade: U

  • Active Managerial Control measures are not satisfactory for establishment to be in compliance with code. No PIC has been designated for the facility. CA: Facility must have a designated person in charge at all times, in order to ensure food safety control.
  • Unable to verify the certified food safety manager for the facility. CA: Informed employees that there must be a current CFSM for the facility and that the CFSM must be the person in charge or designate a PIC when not on the premises. The CFSM must supervise and insure proper handling of food and offer training to food service employees with regard to food safety at the facility.
  • As noted in the previous inspection, was unable to verify that food employees were informed of their responsibility to report food borne illnesses. CA: Discussed and provided the employee health policy to employees, informed employees there must be documentation stating that each employee is aware of this policy.
  • Observed clean utensils stored in a warmer drawer that was not clean to sight and touch. CA: Pointed out to employee, utensils were removed for cleaning.
  • Observed potentially hazardous food (cheese slices, lettuce) cold held at greater than 41 degrees Fahrenheit. See temps below CA: Pointed out to employee, food items were still in the time parameter to be cooled back down, items were moved to the walk-in cooler.
  • Observed bags of ham held past the 7-day discard date. CA: Pointed out to employee, ham was discarded.
  • Observed cups of thousand island dressing with an expiration date of 10/5/18. CA: Informed employee, dressings were discarded.
  • Observed dry storage food stored on rusty shelving. CA: Informed employee that shelving must be smooth and easily cleanable and in good repair.
  • Observed ceiling above prep area with an accumulation of dust. CA: Pointed out to employee and it was stated that ceilings would be dusted and cleaned.
  • Observed excessive grease accumulation under grill and under cooking equipment. CA: Notified employee that areas must be thoroughly cleaned.
  • Observed excessive food debris in oven. CA: Pointed to employee that oven must be thoroughly cleaned to remove old debris.
  • Observed employee items stored in back dry storage area among consumers items. CA: Pointed out to employee, items were removed to a designated area for employee use.
  • Remarks: Provided employee health policy; discussed with employee about the need for more drying space for food containers; and discussed the need for a cooler in the main kitchen area.
  • A follow-up inspection was scheduled, but no date was provided.

  • Unable to verify a certified food safety manager (CFSM)for the facility, as noted in the previous inspection. CA: Informed PIC that a current CFSM certification for the facility must be posted for public view. Manager stated that their certification will be posted as soon as possible.
  • Quaternary ammonium sanitizer not at proper minimum strength in sanitizer buckets. CA: Informed PIC, sanitizer buckets were changed.
  • Observed ice bin in drive-thru not cleaned at a frequency to maintain food-contact surfaces clean to sight and touch. CA: Informed PIC, ice bin was cleaned.
  • Observed beverage dispenser nozzles in drive-thru and dining room not cleaned at a frequency necessary to maintain food-contact surfaces clean sight to touch. CA: Informed PIC, nozzles were removed for cleaning.
  • Observed blender and food containers not clean to sight and touch stored on racks with previously washed food containers and utensils. CA: Pointed out to PIC, items were removed for cleaning.
  • Observed ice cream mixer that was not clean to sight and touch in the front service area. CA: Pointed out to PIC, ice cream machine was cleaned.
  • Observed cheese stored in a container not clean to sight and touch. CA: Pointed out to PIC, container and cheese was replaced.
  • Observed area under hamburger station with an accumulation of food old food debris. CA: PIC stated that area would be cleaned.
  • Observed potentially hazardous food (shredded cheese) cold held at greater than 41 degrees Fahrenheit in prep cooler. See temp below CA: Informed PIC that cheese must be properly submerged in ice bath. PIC removed cheese to walk-in cooler to be cooled down to 41 degrees. PIC stated that time as public health control (TPHC) or an ice bath will be used to monitor temperature or time control.
  • Observed potentially hazardous food (cheese slices) cold held at greater than 41 degrees Fahrenheit near hamburger prep station. An old TPHC label was left up from the previous day. See temp below CA: Pointed out to PIC, cheese was still in the time parameter to be cooled back down to 41 degrees, was placed in walk-in cooler. New TPHC label was made for new cheese slices.
  • Observed ice machine panel that was partially detached from front of ice machine. CA: Pointed out to PIC, it was stated that work would be placed.
  • Observed middle fryer unit unable to drain oil properly. CA: PIC that work order had be placed.
  • Observed prep cooler that did not work and ice baths were being used for food items. CA: PIC stated that work order had been placed for cooler.
