  • Observed previously washed dishes being stacked wet (as noted in previous inspection). CA: Discussed with PIC about adding extra shelving above 3-comp sink (also noted in past a inspection).
  • Observed reach-in cooler in the front kitchen area in need of replacing or repair. CA: Informed PIC that food-contact/non-food contact surfaces must be smooth and easily cleanable.
  • Observed excessive grease build-up on hood vents and in between cooking equipment. CA: Pointed out to PIC and it was stated that areas would be cleaned.

  • PIC failed to demonstrate knowledge by having no priority violations. CA: PIC and employees need training. Will schedule/offer a training.
  • Observed employee come from break room and return to line prep without properly washing hands. CA: Discussed when to wash with PIC.
  • Observed hand washing sink beside 3 compartment with cleaning bottles and scrub brush stored in sink. CA: Discussed sink accessibility and PIC removed items.
  • Observed debris build-up on nozzles of drink machine. CA: PIC instructed employee to clean nozzles.
  • Observed sanitizer bucket beside grill not at proper concentration (min: 50 ppm, observed: 0 ppm). CA: PIC had employee change sanitizer bucket.
  • Observed employees with finger polish/long fingernails not wearing gloves while in the kitchen. CA: Discussed with PIC that employees must wear gloves at all times if they have long/painted fingernails.
  • Observed multiple employees actively preparing food while wearing bracelets or wrist bands. CA: Discussed with PIC that employees cannot wear wristbands or bracelets while preparing foods.
  • Observed wet wiping cloths stored on prep counters. CA: Informed PIC that wet wiping cloths must be stored in sanitizer solution when not in use.
  • Facility does not have a permit displayed. CA: PIC stated that they were under new ownership and they would get a permit.
  • Observed ice scoop stored inside ice machine. CA: Discussed proper storage of ice scoop with PIC.
  • A follow-up inspection will be held Oct. 8.

PAPA JOHN'S PIZZA (Food Service Inspections)*

  • Establishment does not have procedures for employees to follow when responding to vomiting or diarrhea events that involve the discharge of vomit or fecal matter onto surfaces in the food establishment. CA: PIC must have procedures in place to properly respond to a diarrhea or fecal event. Will follow-up with facility. Correct by Sept. 27.
  • Observed encrusted debris build up on can opener. CA: Can opener was removed for cleaning.
  • Observed container of wings with a prep date label from 8/31. CA: Informed PIC about the importance of making sure food containers are labeled with the correct date if held more than 24 hours. Wings were discarded.
  • Observed a pouch of buffalo sauce stored on the back shelf without a date mark (buffalo sauce can be held at room temp for 48 hours). CA: Pointed out to PIC and sauce was discarded.
  • Observed no food service permit displayed for public view. CA: PIC then placed permit in front area for public display.

QUALITY INN (Food Service Inspections)*

  • The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: Provided PIC with more employee health forms and PIC stated that documentation would provided showing that employees are aware of the reporting agreement.
  • Observed potentially hazardous food (boiled eggs, yogurt, and milk) cold held at greater than 41 degrees Fahrenheit. (See temp chart) CA: PIC discarded items, informed PIC that fridge should be turned down and monitored.

  • Observed soil buildup inside ice bin. CA: PIC stated that ice machine would be cleaned as soon as possible. Correct by Sept. 27.
  • Observed several canisters of whipped cream that were past the expiration date of 4/18/18. CA: PIC discarded items.
  • Observed build-up around soda dispenser nozzles. CA: PIC stated that soda machine would be cleaned.

White Columns Kitchen (Food Service Inspections)*

  • Observed a personal drink sitting on a food prep counter. Drink was discarded during the inspection.
  • The hand washing sink was inaccessible in the main kitchen due to pans stored in/on top of sink. Discussed with PIC, pans were removed.
  • None of the three hand sinks in the kitchen and adjacent restroom were provided with soap or paper towels. Discussed at length with the PIC about the need to have soap and paper towels and available at all times at all hand sinks.
  • Observed bulk bags of sugar that were not closed and sealed properly in the waitress wait station. Discussed with PIC. Bag was securely closed during the inspection.
  • Observed beef/macaroni in reach in cooler at 45F. Observed cooked rice in reach in cooler at 45F. Discussed with PIC and these foods were discarded.
  • Observed TCS foods stored in reach in as leftovers without proper date marking of a cook date or discard date. Foods were discarded. Discussed with PIC.
  • Pans of baked chicken placed in reach in for cooling were placed on top of each other preventing proper air circulation which was leading to improper cooling. Discussed with PIC. PIC originally told the inspector the incorrect cook time which was outside of the proper cooling parameters so the decision based on this information was to discard the chicken. Later, after verifying with the cook, the chicken was removed from cooking later than originally thought and would have still been within the cooling parameters which would allowed a quick change in cooling methods to meet parameters but could have prevented the chicken from being immediately discarded. Unfortunately, the chicken had already been discarded at this point. Proper cooling methods and separation of trays was discussed with the PIC.
  • Internal temperature measuring devices were missing from both reach in coolers. Discussed with PIC.
  • Observed the restroom door without a self closing device. Discussed with the PIC.
  • Observed ants within the facility in several different places. Discussed with PIC that a pest control professional should be secured to rid the facility of the ants.
  • Remarks: an informal meeting was called to discuss ownership, permitting and the use of this permitted facility by other caterers.
  • A follow-up inspection will be held Sept. 30.
