Police later identified the woman as Tatyana L. McClain, 25, of Chicago.
McClain offered a price of $140 for half an hour, agreeing to meet at the Courtyard by Marriott hotel on Route 83, according to police.
She then sent the officer a photo of herself in a nun costume, according to the report. She said she would tell the officer where to go when he arrived at the hotel.
Four officers went to the hotel. The officers, who wore black vests with police markings, saw McClain walk into the lobby.
The officer who contacted her asked where her nun costume was. He called her number, and he could see the notification on her phone, police said.
She was put in handcuffs and arrested. She attempted to remove her dress, but an officer prevented her from doing so, police said.
McClain became upset that an officer was holding her phone and pulled away from the police, according to the report. She is then accused of stomping on an officer's foot and kicking the officer's leg backward, police said.
McClain was then taken to the ground.
At the police station, she told an officer that she was going through a tough time and had been trying to rebuild herself, police said. She also said she tries not to have sex with people because she feared the slim chance she would be raped, the report said.
McClain also said the police did a nice job, but was bothered when the officer who she attacked had squeezed her arm, police said.
While in the police station's lockup, she was informed that she was being charged with felony aggravated battery on an officer and prostitution.
In response, McClain got angry and tried to take her clothes off again, the report said.
Eventually, she calmed down and was taken to the DuPage County Jail, police said.
