Martinez said he later found six shell casings at the scene after Thomas discharged his service weapon.
Ascione attempted to treat Simms' wounds, but she complained that she was unable to breathe, the report said.
Inside the residence, the victim's children were awake, with a bloody knife on the end table by the front door, according to the report.
Officer Amanda Benson gave the children snacks while they waited for the mobile crisis vehicle to arrive, according to her report.
Witnesses to the incident during the stabbing who called 911 for help can be heard on the recorded message. One woman said that she was "too scared to watch," but was encouraged by the dispatcher to keep her eyes on the suspect in case he fled.
