Beverly Public Library: 2021 Tax Forms

The main library and Farms branch will make federal and state tax forms available as soon as they arrive. Quantities may be limited and are determined by the IRS and MA Department of Revenue. We are unable to hold forms, they are available on a first come, first served basis.

MA DOR tax forms and instructions

You may order federal forms by mail through the IRS or call 1-800-829-3676. To request MA forms call the Department of Revenue at 1-800-392-6089.
