  • Submission of the form DOES NOT guarantee a test kit.
  • Only ONE form submission per household, please. Duplicate form submissions with the same IP address WILL BE DELETED.  
  • There are two tests in each test kit. 
  • A maximum of two test kits (four tests total) will be provided per household.
  • Test kits will be provided to Framingham residents ONLY. You will be REQUIRED to show proof of residency.
  • Residents will be able to choose from one of two collection days.
  • Face coverings are REQUIRED when collecting test kits. 
  • If inclement weather occurs, the distribution will be rescheduled.
