We are pleased to share the following information regarding COVID-19 vaccination clinics that are being offered in collaboration with the Central Massachusetts Regional Public Health Alliance. While these clinics have been scheduled primarily to provide vaccines for our students ages 5 -11 years old, other eligible family members are welcome to attend to receive vaccinations (first, second, or booster doses) with their children.
WHERE:Â Oak Middle School cafeteria (45 Oak St - please park at the at the front of the building and enter using the auditorium entrance doors on the left, then proceed straight back to the cafeteria)

DATES:Â Tuesday 11/30, Wednesday 12/1, Thursday 12/2Â
    Monday 12/20, Tuesday 12/21, Wednesday 12/22Â

TIME:Â 4:00 PM - 7:00 PM

TO REGISTER: Pre-registration for a time slot is required separately for each individual who will be getting the vaccine. Please use this link, choose COVID-19 vaccine, complete the dose and date of birth information, then continue and choose the clinic named Shrewsbury Public Schools ONLY (Family). Appointments are available on a first-come, first-served basis. Please register as soon as possible to secure your spot.

Anyone receiving a second dose or booster vaccine should bring their COVID vaccination card with them to the clinic. Moderna, Pfizer and J&J vaccines and boosters will be available (Pfizer only for individuals ages 5 - 17).
