Free COVID Vaccine Clinic To Be Held For Warren Area Residents
Middle-Brook Regional Health Commission and Green Brook Family Medicine will be providing Moderna and Pfizer vaccines on Monday.
WARREN, NJ — The Middle-Brook Regional Health Commission and Green Brook Family Medicine will be providing COVID vaccines on Monday.
When: Monday, Jan. 10 from 2 to 4 p.m.
Where: Green Brook Municipal Building, 111 Greenbrook Road

Registration: To assure adequate supplies for everyone attending registration for the COVID vaccine is required through the following website: mbrhccovidvax.timetap.com.
At the Clinic:

- Bring insurance cards to the clinic for everyone that has insurance. The vaccine is free of charge.
- A parent/guardian is required to be present with their child.
- Dr. Frank or Dr. Cook will be available at the clinic to address any COVID vaccine-related questions.
- Both the Moderna COVID Vaccine/Booster for adults ages 18 and up as well as the Pfizer COVID Vaccine/Booster for ages 12 and up will be available. Pfizer vaccine for ages 5 to 11 will also be available.
