City officials reported one death and three serious injuries as a result of the crash, which took place at 3:24 p.m. off E. 34th Street in Manhattan as the aircraft attempted to return to the helipad shortly after liftoff. Though more questions than answers accompanied this latest crash—the second fatal city helicopter incident to occur in just over two years—here's one question on some New Yorkers' minds:
