Yorktown Offering Senior I.D. Cards To Residents
For those age 60 and up, the cards and forms may be useful in the event of an emergency, providing valuable information quickly.
YORKTOWN, NY — The town of Yorktown is providing a new program that will benefit its senior citizens.
Diana Quast, the town clerk, announced Monday the Senior Identification Program which will be made available to all seniors age 60 and up in Yorktown.
Senior ID cards will be provided by the clerk’s office to Yorktown residents complete with a resident’s personal data, physician’s contact information and emergency phone numbers. Allergies and other vital information can also be printed on these wallet-sized cards.

The cards and forms may be useful in the event of an emergency because medical emergency services can quickly gather pertinent information about a senior and contact his or her doctor and family members.
Quast said that it was important to note that the applicant is the only one with the record when the card is completed. The clerk’s office does not keep any data.

Apply for the ID card online under Senior ID Form Application, through this direct link or by appointment at the clerk’s office.
Seniors can also apply through the Town Clerk’s Mobile Office, which regularly travels to senior facilities to provide services to residents.
Senior clubs and other organizations may also call the clerk’s office at 914-962-8152 to set up their town dates and times that the mobile office can come to them.
