Stark County Health Department Will Provide Vaccination Record From State System In Place Of New Card

(Jackson Twp., OH) – The Stark County Health Department will begin offering vaccination records in place of replacement COVID-19 vaccine cards. Those residents who have misplaced their COVID-19 vaccine card will now receive a printed version of their vaccination record from the Ohio Impact Statewide Immunization Information System (ImpactSIIS) Web application. Residents may call the department at (330) 451-1681 to speak with a staff member about retrieving a copy of the record. The health department asks residents to pick up the document at 7235 Whipple Ave. NW, North Canton, Ohio 44720 with a state issued ID.

“We have decided to provide the full immunization record for those needing proof of their COVID-19 vaccination,” said Health Commissioner Kirkland Norris, RS, MPH. “Any vaccinations that a resident has received in the past, and was entered in ImpactSIIS, will also be listed on the record.”

The COVID-19 vaccine cards are still usable when needing to show proof of vaccination. The Ohio Department of Health expresses that residents should take a picture of their card and save it on their smartphone. Residents who received their vaccine with the Stark County Health Department, may also access their information through the ArmorVax platform. The mobile app is available for both Android and Apple devices. Those who received their vaccine at another provider, should call that provider if the vaccine record needs replaced.
“Our county is close to reaching 60% of eligible residents with at least one dose of the vaccine. That milestone will make way for healthier days ahead for our community,” said Norris.

Those residents wanting to schedule an appointment for their COVID-19 vaccine (those 12 years of age and older) can schedule through ArmorVax.com and on the mobile app for Apple and Android devices. Those that are minors, 12 to 17 year old’s, must be accompanied by their parent or legal guardian to their vaccine appointment. The parent or legal guardian must sign a consent form upon checking in at the clinics. However, those that do not have access to a computer, internet or who have difficulty using ArmorVax, may still call the registration phone line at 330.451.1681 to schedule an appointment.
