Pizza Shop Could Not Prove Staff Trained On 'Big 6' Foodborne Illnesses
Rappahannock Area Health District inspectors recently visited 21 Fredericksburg food establishments and found violations at eight of them.
FREDERICKSBURG, VA — Rappahannock Area Health District inspectors reported finding a combined 36 violations at eight of the 21 Fredericksburg food establishments they visited April 27-May 7.
At the Domino's Pizza on Emancipation Highway, an inspector reported that the person in charge could not show verification that all current employees have been trained on "The Big 6" foodborne illnesses and their common symptoms.
“The Big 6” pathogens (Norovirus, Nontyphoidal Salmonella, Salmonella Typhi, E. coli, Shigella, and Hepatitis A) are highly infectious, and can able cause severe disease in small quantities, according to the Michigan State University Extension.

"The person in charge was given a new sample employee health policy to be reviewed, signed and dated by all employees," according to the inspection report.

During the inspection the person in charge relocated all equipment to the three compartment sink to be cleaned.

- Observed three hoses with spray nozzles connected to the water line with a hose bibb backflow preventer. Instructed the operator a dual check valve with atmospheric vent backflow preventer is required. Backflow prevention handout was provided during preopening inspection conducted on 4/21/2025.
- Corrective Actions: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bibb (threaded faucet) if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by law by installing an approved backflow prevention device as specified under 12VAC5-421-2210.

- Observed two packages of thawed raw tuna inside ROP in the sushi cooler at 40 -41 degrees . EHS discussed with the person in charge removing the tuna from the ROP or cuting a opening before defrosting. Food was voluntary discarded.
- Observed on the sushi menu red snapper offered for sale. Through discussion with the person in charge the facility is currently using tiliapa in place of the red snapper . EHS informed the person in charge the food must be honestly informed on the menus. During the inspection the person in charge changed the menus.
- Observed ceiling tiles in the sushi bar and server area are not smooth and easily cleanable.

- Observed several stainless-steel containers directly touching uncovered foods in the prep unit on the cookline. Containers were removed. Corrective Actions: Food shall only contact surfaces of: 1. Equipment and utensils that are cleaned as specified under 12VAC5-421-1770 through 12VAC5-421-1860 and sanitized as specified under 12VAC5-421-1885, 12VAC5-421-1890 and 12VAC5-421-1900. 2. Single-service and single-use articles. 3. Linens, such as cloth napkins, as specified under 12VAC5-421-560 that are laundered as specified under 12VAC5-421-1920 C.
- Observed raw foods over RTE foods in the WIC. Corrective Actions: Food shall be protected from cross contamination by: 1). Except as specified in subdivision 1 d of this subsection, separating raw animal foods during storage, preparation, holding, and display from: a. Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables. b. Cooked ready-to-eat food. c. Fruits and vegetables before they are washed. d. Frozen, commercially processed, and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food.
- Per discussion with the person in charge, the food thermometer is only being cleaned with alcohol wipes. Corrective Actions: Equipment food-contact surfaces and utensils shall be cleaned at any time during the operation when contamination may have occurred.
- Observed the prep cutting boards stained and scratched. Corrective Actions: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
- Observed no consistent datemarking system of RTE/TCS foods in the WIC. Corrective Actions: Except as specified in subsections E, F, and G of this section, refrigerated ready-to-eat, time/temperature control for safety food prepared and packaged by a food processing plant shall be clearly marked at the time the original container is opened in a food establishment and if the food is held for more than 24 hours, to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, based on the temperature and time combinations specified in subsection A of this section and. 1. The day the original container is opened in the food establishment shall be counted as day 1. 2. The day or date marked by the food establishment shall not exceed a manufacturer's "use by" date if the manufacturer determined the "use by" date based on food safety.
- Observed wet stacking on shelving over the 3comp sink. Corrective Actions: After cleaning and sanitizing, equipment and utensils: 1. Shall be air dried or used after adequate draining as specified in 40 CFR 180.940 before contact with food. 2. Shall not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
- Observed the exterior lid of the grease receptacle heavily soiled. Corrective Actions: Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil or becoming attractants for insects and rodents.
- Observed the following in need of cleaning: 1) floors underneath the equipment. 2) ceiling vents/tiles. 3) walls. Corrective Actions: The physical facilities shall be cleaned as often as necessary to keep them clean.

- Observed a long black hose connected to the facuet at the wall with a spray nozzel attached and absent of a dual check backflow preventer. During the inspection the person in charge removed the spray nozzel attachment . EHS provided a handout.
- Observed walls,floors and ceilings ( including air vents ) in need of cleaning.

What Violations Do VDH Inspectors Report?
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
Sometimes, the follow-up inspection only considers the most important "priority" violations.For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."
