26 Found Violations At Restaurant: Woodbridge Restaurant Inspections

WOODBRIDGE, VA — Health inspectors found up to 26 violations at some restaurants during recent inspections in the Woodbridge area.
Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually. Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- The ice machine is in disrepair. There is a build up of ice under the compressor in the walk-in freezer, inside unit and per the CFPM the walk-in freezer, outside unit is not working properly (only bags of ice stored in this unit currently).
- There are no test kits located in the facility for monitoring the concentration of the chemical sanitizing solutions (quaternary ammonium at the 3 compartment sink and chlorine at the warewashing machine).
- The waste lines (all 3) from the 3 compartment sink extend into the floor drain.

- The Person In Charge (PIC) was not able to provide documentation that all food employees (approximately 10) or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
- This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
- Open drinking containers were stored (on the food prep table/cutting board on prep refrigerator) in a manner that may contaminate food, food contact surfaces or utensils.
- There was no soap available at the small handwashing sink in the kitchen.
- There were no dispensed disposable hand towels available at the following handwashing sinks: the handwashing sink in the kitchen and the handwashing sink in the bar
- Signage that notifies food employees to wash their hands was not provided at the handwashing sinks used by food employees in the kitchen, at the bar and in the women's and men's toilet rooms.
- In the Turbo Air 4 drawer prep reach-in refrigerator, raw chicken was stored over raw chorizo (pork) and raw beef was stored over raw pork. In the Frigidaire 1 door upright freezer, raw chicken (in secondary plastic wrap) was stored over raw shrimp (in secondary plastic wrap).
- Fried chicken, cooked on 7/28/2025 at 8:00pm (per PIC), was found covered in walk-in cooler at 73 degrees F at 11:15am on 7/29/2025.
- Large containers (in volumes too great to be used in a single day) of cooked shredded chicken and carrots, ground beef, rice, shrimp and chicken (that were cooked on 7/26/25 or 7/28/25 per the PIC) were not marked with the date to be discarded.
- Per the Person In Charge (PIC), only the Tilapia fish used in the ceviche is served raw (treated by lime only).
- The concentration of the chlorine solution in the wet wiping cloth bucket was measured using the EHS's test kit and found to be above the desired concentration @ 200+ppm (toxic). This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
- Fried chicken in walk-in cooler observed to be cooling improperly in deep container and covered with plastic wrap.
- There were no temperature measuring devices (thermometers) located in the following cold or hot holding equipment: chest freezer (left); Frigidaire 1 door upright freezer, chest freezer (right), or any of the refrigerators in the bar area.
- Various foods along the cookline and other foods in blue bulk storage containers were not labeled properly with the common name of the food.
- Foods observed to be stored on the floor in the walk-in cooler.
- Taquitos and various foods in the walk-in cooler found to be uncovered and exposed to potential contamination. Various foods in the Frigidaire upright freezer were found uncovered.
- The wet wiping cloths on the counter in the kitchen and on the bar were not properly stored in the sanitizer bucket in between use.
- Single service carryout containers were stored on a top shelf with their food contact surfaces directly exposed to potential contamination.
- Single service containers such as foil pans and plastic (soup?) containers were washed and ready for re-use above the 3 compartment sink.
- Various single use containers, such as the Daisy sour cream and ice cream containers, are being reused to store other foods in throughout the kitchen.
- Wooden utensils were available in the kitchen to stir foods while cooking.
- Milk and vegetable crates are being used throughout the kitchen and walk-in cooler for the storage of foods and as shelving.
- Raw wood is being used in various places in the kitchen such as under the rice cooker and in the bar area. Cardboard is being used to line shelving throughout the kitchen and in the walk-in cooler.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (chlorine).
- Black connections have been added to the original wastewater pipes of the 3 compartment sink so that the original air gaps were eliminated and the continuous pipes now extend down into the floor drain/grease trap.
- The wet mop was observed to be stored down in the water in the mop bucket.

- Food employees are not informed in a verifiable manner of their responsibilities to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.
- There are no clean up procedures present for employees to follow in response to vomiting and diarrhea events.
- Observed the reading thermometer on the outside surface of the walk-in cooler was not functioning properly and is broken. In addition, it did not match the temperature inside the walk-in cooler.
Burger King #5463: 2211 Tacketts Mill Dr Woodbridge, VA 22192

Violations:
- Many of the handwashing sinks throughout the food establishment were found to have hot water at less than 100 degrees F.
- Sliced tomatoes, in covered containers in the walk-in cooler and date marked Tuesday (today) were found at 51 degrees F.
- There are no ambient air monitoring devices (thermometers) in the chest or reach-in freezers throughout the food establishment.
- The food establishment is under construction (exterior) and has only a tarp (no wall) on one side of the building. The food establishment is closed to dine in service, but still continues to cook and serve food through the drive thru windows. Many flies were observed in the food establishment.
- Cups up front are stored on a tray and not protected from contamination.
- The door to the walk-in freezer still does not seal properly resulting in a build up of ice along the edge of the door.
