Early Bird Registration for Boys & Girls ages 4 through 18

Saturday, Nov. 12th

SUPER EARLY BIRD SALE

1 Day Only — $30; 10 a.m. to 2 p.m. at the Ashland Little League field (snack bar), behind the skate park (16301 E. 14th St., San Leandro)

Requirements for $30 special:

• All forms must be completed
• fee paid in full
• must have all proofs of residency with original birth certificate for the player and parent/guardian photo ID by Nov 12th.

Acceptable Proofs of Residency are Three of the Following:

• Driver’s License Voter’s Registration School Records Welfare/Child Records
• Federal Records State Records Local (municipal) records Support payment records
• Homeowner or tenant records Insurance documents Medical records Military records
• Vehicle records Employment records Internet, cable, or satellite records (ie.Comcast/DirecTv)
• Utility bills (ie gas, electric, water/sewer, phone, mobile phone, heating, waste disposal)
• Financial (loan, credit, investment, etc) records

Please note that all proofs of residence need to be dated September 1, 2011 or after.

REGULAR "EARLY BIRD" SIGN UP

3 Days Only — $50 per player.

• Sunday, Nov. 13 10 a.m. to 2 p.m.
• Saturday, Nov. 19 10 a.m. to 2 p.m.
• Sunday, Nov. 20 10 a.m. to 2 p.m.

Also at the Ashland Little League field (snack bar), behind the skate park.

Again, all the forms need to be filled out on those days and submitted with proof of age for the participant, 3 proofs of residency and full payment.
