Community Clinic Flyer Community Clinic Flyer There will be a free, walk-up COVID-19 Vaccination Clinic, for 5-11 year olds, at Newington High School, in the cafeteria, on November 30 from 4:00PM - 7:00PM specifically for our Newington students! For your convenience, please fill out this survey to make your child’s appointment time (walk-ins are welcome!). The Connecticut Department of Public Health in collaboration with vaccine providers are bringing this clinic to our community to make the COVID-19 vaccine more accessible.  Note - the second vaccine will be given on 12/21/21 at the same place and time. this survey Once you have registered, please print and complete the 2 page form and bring with you to your student’s appointment: COVID-19 Vaccination Intake Form (2 pages) Please bring your student’s health insurance card and photo ID if you have them. Note, it is not necessary to have health insurance or ID to receive the vaccine. No one will be turned away for lack of insurance or ID. Newington School District encourages vaccination for everyone who is eligible. This truly lifesaving vaccine is the best protection for you, your family and your community against the potentially devastating effects of the COVID-19 virus.
