  • Observed raw animal foods not properly separated by cook temperatures. Observed raw eggs stored touching raw chicken. CA: all foods were separated according to cook temperature.
  • Observed food items stored in reach-in freezer outside of containers. Also observed flour stored uncovered. CA: PIC will ensure that all items are covered. Re-check on Nov. 20.
  • Observed food debris build-up on inside of ice machine. CA: PIC will clean ice machine. Re-check Nov. 20.

  • Observed containers of raw chicken and shrimp stored over raw shrimp and pork in the walk-in cooler. CA: Discussed proper separation with PIC, food items were rearranged.
  • As noted on a previous inspection, several containers stored in the walk-in cooler without coverings or either the plastic covering has fallen down into the food. CA: Discussed with PIC, items were covered.
  • Observed several bottles at the grill that were not labeled. CA: Informed PIC that once food items are removed from the original container then new containers must be properly labeled.
  • Observed wooden boards used to store food containers on in the walk-in cooler. CA: Informed PIC that food and non-contact surfaces must be smooth and easily cleanable.
  • Observed food waste build-up under prep counters in the main kitchen. Observed dry storage shelving in need of cleaning.CA: Pointed out and informed PIC that a lot of miscellaneous items that were not used should be removed from kitchen and dry storage areas.
  • Observed employee keys and glasses on the prep counter. CA: Discussed using a designated area for employee use. Items were removed.
  • Observed containers of employee food items mixed in with consumer items. CA: Discussed designating an area for employee food.

  • Observed unauthorized persons (child) in food service area. CA: Discussed with PIC that children cannot be in food service area.
  • Observed that certified food safety manager did not ensure that person in charge had sufficient knowledge of foodborne illness risks as evidenced by above violation. CA: CFSM will reassess the training needs and retrain accordingly.
  • Observed employee beverages in non-single service cup with lid and straw. CA: Discussed with PIC that all employee beverages must be in single service cup with lid and straw.
  • Observed dishwasher sanitizer at less than the manufacturer's recommended concentration (0 ppm). CA: Discussed replacing sanitizer bottle with PIC.
  • Observed recently washed dish with grease residue on it. CA: Pointed out dish to PIC for rewashing.
  • Observed food debris buildup on microwave and slicer. CA: Discussed proper cleaning frequency and method with PIC.
  • Observed single service condiment cups being used as scoops for spices rather than scoops with handles. CA: Discussed the use of proper scoops with PIC.

  • Observed raw beef stored over raw fish in walk in cooler. Observed open containers of chicken stored over open container of carrots in walk in freezer. CA: PIC rearranged foods in cooler and walk in freezer according to cook temperature.
  • Observed encrusted food on inside of microwave oven. CA: Discussed proper cleaning method and frequency with PIC.
  • Observed line cook wipe knife with sanitizing cloth without properly washing, rinsing, and sanitizing knife before use. CA: PIC took knife to be properly washed, rinsed, and sanitized.
  • Observed sanitizer bucket with no detectable amount of sanitizer in the solution (0 ppm). CA: PIC removed sanitizer bucket and refilled it with new sanitizing solution.
  • Observed potentially hazardous foods (lettuce, liquid eggs) not stored at 41 degrees Fahrenheit or below. CA: Lettuce had been out for less than four hours and was placed in reach in cooler to cool. PIC chose to discard eggs.
  • Observed previous inspection not displayed in facility. CA: Provided PIC with copy of current inspection.
  • Observed no food permit displayed in facility. Observed ServSafe certificates not displayed where customers can properly view them. CA: Discussed with PIC that food permit and certified food safety manager certificates must be displayed where customers can easily view them.
  • A follow-up inspection will be held Dec. 4.

  • Observed bottles of sauces by the grill area without labels. CA: Informed PIC that bottles must be labeled when removed from the original container.
  • Observed potentially hazardous food (shrimp) thawing in a bucket of standing water.CA: Pointed out to PIC,PIC then moved shrimp to sink with running water.

  • Observed potentially hazardous food items in walk-in cooler (baked beans, mac and cheese, potato salad) not properly date marked. CA: Food items were discarded.
  • Observed bowls and cups used as scoops in cinnamon sugar mix and sugar containers. CA: PIC removed items. Discussed proper scoops and that they should be stored handle up.
  • A follow-up inspection will be held, but no date was provided.

Informed PIC that food must cool from 135 degrees to 70 degrees Fahrenheit within 2 hours, then 70 degrees to 41 degrees Fahrenheit within 4 hours. Also discussed portioning cooling foods into smaller shallow pans.
  • Observed employee drink cups (yeti, coffee cup) stored near wing sauces. CA: Informed PIC that employee drinks must be stored in a disposable cup with lid and straw and kept in a designated area for employee use. Cups were removed.

  • Observed canisters of whipped cream that had expired on 11/6/18. CA: Notified PIC, whipped cream was discarded.

  • Observed that PIC could not demonstrate sufficient knowledge of foodborne illness risks as evidenced by priority violations including proper food storage and separation, proper cold holding temperatures, food contact surfaces cleaned and sanitized, and proper separation of toxic substances. CA: CFSM will reassess needs of PIC and retrain accordingly.
  • Observed that certified food safety manager did not ensure that PIC has sufficient knowledge of foodborne illness risks as evidence by the priority violations. CA: CFSM will reassess the training needs of employees and retrain accordingly.
  • Observed shells eggs stored over hash browns. Observed raw chicken stored over raw beef. CA: PIC rearranged food in cooler. Discussed installing a barrier in the cooler so that eggs would not be pushed over above hash browns.
  • Observed mold-like building up in ice machine and on soda nozzles. Discussed proper cleaning methods and frequency with PIC. Repeat Violation. Correct by Nov. 27.
  • Observed that the final rinse on the high heat dishwasher did not reach 180 degrees Fahrenheit. CA: Dishwasher cannot be used until it is repaired and checked by health department. Dishes, utensils, and equipment must be washed, rinsed, and sanitized in the three compartment sink. Please call health department when dishwasher is repaired. Rechecked on 11/16/18. Dishwasher was functioning properly.
  • Observed potentially hazardous foods (raw beef, uncooked bacon) held at temperatures above 41 degrees Fahrenheit. CA: Breaker was flipped in walk in cooler and food was properly cooling. Food was held at temperature for less than four hours and was allowed to cool. Walk in cooler will be rechecked at re-inspection.
  • Observed bottle of sanitizer stored over food. CA: Sanitizer was moved to bottom shelf.
  • Observed buildup of oil and grease in drawers below grill. Observed food debris buildup and rust on shelving in reach in cooler. CA: Discussed proper cleaning frequency and repair of shelving to an easily cleanable surface with PIC. Remarks: Rechecked dishwasher on 11/16/18. Dishwasher was functioning properly.
  • A follow-up inspection will be held Nov. 27.
