Violations: 4-2B, Violation of Code: [.05(7)(a)1 ] 1. Ice machine unclean with a build-up of mold on the underneath portion (food contact portion) that is above the wheeled ice bins. 2. CFSM had employee clean the ice bin before the end of the inspection. 3. Ice machine must be cleaned at a frequency necessary to preclude accumulation of soil or mold. Corrected On-Site. New Violation. 14A, Violation of Code: [.04(4)(k) ] 1. Scoops for cinnamon sugar and flour were being stored with the handles lying down in the food.2. CFSM moved the scoops so that the handles were above the food and the containers during inspection. 3. While not in use, food preparation/dispensing utensils must be stored with their handles above the top of the food and the container. Corrected On-Site. New Violation. 14C, Violation of Code: [.05(10)(f) ] 1. Plastic drink lids in self-service drink area being stored improperly with the food contact portion of the lid facing upward, exposed to customers. 2. Please turn the plastic drink lids inverted to prevent risk of contamination. 3. Single-service articles, for self-service, must be stored properly to prevent contamination. New Violation. 17C, Violation of Code: [.07(5)(a),(b) ] 1. Walls in front prep area behind pretzel making area were unclean with old food debris; walls in back behind storage large bags of popcorn kernels and other food were unclean with old food debris and mold. 2. Please include cleaning of walls to regular cleaning schedule.
3. Must keep the walls in food prep and storage areas clean to prevent the build-up of old food debris and/or mold. New Violation. 17D, Violation of Code: [.07(2)(i) ] 1. Lights in mop room and chemical storage room were unshielded; the light panel was missing and light bulbs were not shatterproof. 2. Replace light cover or provide shatterproof bulbs in this area. 3. Light bulbs must be shielded with a protective shielding that can include a shield or shatterproof bulbs. New Violation.

Violations: 6-2, Violation of Code: [.04(6)(g) ] 1. Refrigerated, ready-to-eat, potentially hazardous foods were not date marked as required. The chili that had been removed from sealed can and placed in the WIC was not date marked. 2. PIC correctly put a date on the ready-to-eat, potentially hazardous foods that required date marking. Remember to date mark the packages of cheese and milk once the manufacturer's seal is broken. 3. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours must be clearly marked to indicate the date by which the food must be consumed, sold, or discarded. In-use, wet wiping cloths not being stored in sanitizer; wet wiping cloth being stored in empty bucket. This is a repeat violations from inspection on 4/30/12. 2. Wet wiping cloths were put in appropriate sanitizer during inspection. 3. In-use, wet wiping cloths must be stored in an approved sanitizing solution (chlorine or Quat). Corrected On-Site.
