- There was no person in charge at the start of the inspection. Designated a person in charge There must be a person in charge on the premises of the food service establishment at all times.
- facility could not provide verifiable way to show that employees had been trained on symptoms and illnesses to report to management Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food.
- Designated person in charge could not relay five symptoms, six illnesses provided paperwork and reviewed all information with person in charge. Person in charge shall explain their responsibility for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease and can describe the symptoms associated with the diseases that are transmissble through food. They also shall be able to explain how food employees and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees.
- Observed no soap at the hand washing sink soap provided Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
- None of the handsinks in the facility were reaching a temperature of 100F A handwashing sink shall be equipped to provide tempered water at a temperature of at least 100ºF (38ºC) through a mixing valve or combination faucet.
- Person in charge was unaware of what was being used for body fluid clean up kit. Facility does have bleach and written procedures. A food establishment shall have procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.
- Observed mold like build up on interior of ice machine Equipment food-contact surfaces and utensils shall be clean to sight and touch.
- Observed no temperature measuring device within the facility. Food temperature measuring devices shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
- Observed live roaches in an open bag of flour Roaches killed, flour discarded. "The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1) routinely inspection incoming shipments of food and supplies; 2) routinely inspecting the premises for evidence of pest; 3) having a professional treat the facility; and 4) eliminating harborage conditions."
- Observed both the front and back doors being propped open. Both doors closed. "Outer openings of a food service establishment shall be protected against the entry of insects and rodents by: 1) filling or closing holes and other gaps along floors, walls, and ceilings; 2) closed, tight-fitting windows; and 3) solid self-closing, tight-fitting doors."
