Greater Family Health Expands Access to Quality Care with New Health Center in Des Plaines, IL
The new Health Center is set to open on August 18th, 2025.

Greater Family Health, the 5th largest Community Health Center in the state of Illinois, has announced the upcoming opening of its 12th Community Health Center, located in Des Plaines, Illinois.
Scheduled to open in August 2025, the new facility located at 1515 Lee St., will be made up of 12 state-of-the-art examination rooms, a full-service LabCorp laboratory, and a CLIA-waived lab for point-of-care testing.

The new Health Center in Des Plaines will initially offer Family Medicine, Pediatrics, Obstetrics and Gynecology services.

Care will be provided by four practitioners including two experienced Greater Family Health Doctors, Dr. Anh Si Le, DO, and Dr. Cheerag Patel, DO. Joining them will be two new providers, Alicia Schanbacher, MSN, APN, FNP-C, a true professional with over 20 years of nursing experience, and Fabiola Martinez, MSN, FNP-C, a dedicated Greater Family Health employee since 2014 who has advanced through multiple roles in the organization. All four are highly talented practitioners and will be a valuable asset to the areaβs residents.
This new Health Center will be in good company, joining our network of 11 Community Health Centers, providing a comprehensive range of high-quality, affordably priced, fully integrated primary care services to over 76,000 area residents via 246,000 medical, behavioral, and oral health appointments annually across five counties in Northeastern Illinois. All centers are accredited by The Joint Commission, are Primary Care Medical Home certified and nationally recognized for both health outcomes and cost efficiency. Greater Family Health provides care to all, regardless of the patientβs ability to pay.
