Samuel Cizek age 89, beloved husband of the late Esher nee Grossman; loving father of Bashi (Roger) Packer, Gerry (Barbara) and Sherry (Philip) Magid; adored zaide of Zehava (Ophir) Agassi, Michael IRachel) Packer, Laura (Yekutiel) Aloni, Stephanie (Yaakov) Rothstein, Cory Cizek and Molly Cizek; cherished greatgrandfather of Anabel Packer; dear uncle of many. Holocaust survivor.

Service Monday at noon at Chicago Jewish Funerals, 8851 Skokie Blvd (at Niles Center Road), Skokie. Interment New Light Cemetery.
