JAG Physical Therapy Commemorates Hazlet Facility with Ribbon Cutting Ceremony
Hazlet Mayor, Local Physicians and Patients Attended in Celebration of Newest Monmouth County Location
Bridgewater, N.J. (October 28, 2025) – JAG Physical Therapy, one of the fastest growing comprehensive physical and occupational therapy practices providing rehabilitative care to patients and sports teams in over 170 locations across the Northeast, recently held a formal ribbon cutting event to celebrate its Hazlet facility.
Recently relocated to 3047 NJ-35, Hazlet, NJ 07730, this Monmouth County establishment marks JAG Physical Therapy’s 12th facility within the county. To celebrate this newest clinic, several members of the Hazlet community attended the event, including Mayor Michael Sachs, local physicians and JAG Physical Therapy patients and team members. The MAX Fitness Challenge of Hazlet also had a presence at the event, and held a table to share information about their fitness programs and community services with attendees.
Mayor Sachs offered his support for the company’s new facility, stating, “We are all here for you in Hazlet; I am here, the town is here. We wish you continued success and hope you are here for many years to come!”

Michael Wylykanowitz, PT, DPT, Regional Director of JAG Physical Therapy, shared his sentiment about welcoming patients into the clinic: “JAG Physical Therapy is built on helping our local communities and working to get patients better. Hazlet Clinical Director Nick Arcabascio, PT, DPT, reflected on how meaningful JAG has been in his career, sharing: “I've been able to grow my career at JAG for the last two and a half years. The Hazlet facility offers personalized care and specialized treatment plans to address various health concerns and provide accessible, comprehensive care for community members. Patients who visit the Hazlet clinic do not need a referral from a doctor to begin physical therapy treatment and can work with a therapist for up to 30 days before needing a referral to continue services.
