- Available to Chamber Members with as few as 2 eligible employees working 24+ hours/week. While Sole Proprietors (Schedule C) are not eligible, some Husband & Wife Only (W-2) and Partner Only (K-1) businesses may be eligible for coverage;

- Access to CentraState’s Community Based Network (CBN) and the QualCare Regional HMO Network, one of New Jersey’s most comprehensive provider networks with the added benefit of access to Memorial Sloan-Kettering in New York and a number of major Philadelphia area hospitals;
- Nationwide network access outside New Jersey through the First Health National PPO Network;
- The CCHP can be offered alongside any or all of the Affiliated Physicians and Employers Health Plan Trust’s twelve other plan options including: PPO, Open Access POS, Network Only and HSA/HRA Compatible alternatives;
- Features convenient composite rating (unlike the age-banded format used by other carriers) and a focus on Wellness, Disease Prevention and Coordination of Care;
- COBRA/NJ Continuation administration is included at no cost to the employer and HRA/FSA administration is available with no monthly cost and an annual set-up/renewal fee of only $250;
- Member advocacy services and a wide variety of competitive Dental, Vision, Life, Long Term Disability and Voluntary Benefit programs available through the PIER Practice Solutions / CBIZ marketing partnership.
