Village Of Freeport: Update For FSA - DCA 2022

Flexible Spending Account/ Dependent Care Account: If you wish to enroll / re-enroll in the FSA/ DCA plan please complete the attached form and submit to Human Resources directly, or by emailing the completed form to HRHELP@FREEPORTNY.GOV (This is completely voluntary). In order to receive your FSA/DCA card & funding by 1/1/22 you must submit the FSA-DCA enrollment form by 12/13/21.

The yearly amount for 2022 has increased to $2,850. If you wish to contribute this amount please write it in. If you have already submitted your form and would like to increase your amount please let me know.
